License f;o. 25-07596-02 flotice of Violation Based on the results of the fiRC inspection conducted on f;ovcaber 30, 1978, certain activities appear to be in noncompliance with flRC regulations and the conditions of your f1RC License !!o. 25-07596-02 as indicated below:
1.
Contrary to License Item 8.A., which authorizes a maximum possession of two sources of 8 millicuries each of c.esium-137, one source of 8.9 millicuries of cesium-137 was possessed on fiovember 30, 1978.
This is an infraction.
2.
Contrary to License Condition 13. A.(2), one cesium-137 sealed source containing 8.9 millicuries was not always tested for leakage within six months prior to the date of use during the period I;ovember 12, 1976 to fiovember 30, 1978.
This is an infraction.
3.
Contrary to License Condition 12, licensed material was not always used in the physical presence of an authorized individual during the period August 23, 1978 to October 1,1978.
This is an infraction.
4.
Contrary to 10 CFR 20.401(a), records were not maintained of personnel monitoring purportedly supplied during the period July 1, 1977 to October 1,1977 in accordance with 10 CFR 20.202(a).
This is a deficiency.
5.
Contrary to 10 CFR 20.401(b), records were not maintained of the survey purportedly made to demonstrate compliance with 10 CFR 20.105(b) radiation levels in the unrestricted area exterior to the vehicle used for transport of licensed material This is a deficiency.
6.
Contrary to 10 CFR 19.11(a) or (b), neither the documents required by 10 CFR 19.ll(a), nor the notice required by 10 CFR 19.ll(b) were posted on fiovember 30, 1978.
This is a deficiency.
7.
Contrary to 10 CFR 19.11(c), Form flRC-3, "flotice to Employees," was not posted at the restricted area on l'ovember 30, 1978.
This is a deficiency.
