Dear Ms. Lange:

This is in answer to your letters of December 16 and 19,1978, which were forwarded to this office for reply, and is in additi n to our phone conversation of December 26, 1978, concerning the concrete problem in the Wolf Creek containment building wall.

Voids in the concrete were observed by Daniel workmen and Quality Control inspectors when the forms were removed from the sixth lift in the containment building wall on December 13, 1978. The on-site KG&E representatives received prompt notification and KG&E corporate management in turn notified the NRC on December 14, 1978.

The situ-atica was common knowledge on the site and was communicated to the news media. When a reporter from TV station WIBW Topeka, requested permission to take pictures, KG&E consented.

NRC inspector., were on site December 18 and 19,1978. The inspectors confirmed that there were two voids in the sixth lift of the contain-ment building wall. The voids were observed to be under the equipment and personnel hatches. Both voids are considered repairable with no degradation of strength of the wall. The inspectors' initial findings are contained in inspection report No. STN 50-482/78-15, which should be available sometime after mid January in the Local Public Document Room, located in the Coffey County Courthouse in Burlington. We will send you a copy as soon as it is available.

On December 18, 1978, representatives from the corporate management of Kansas Gas & Electric Company, Kansas City Power and Light Company and Daniel International met with the Region IV director and members of his staff to discuss corrective action in regard to the problem. The dis-cussion focused on actions to prevent recurrence.

Ms." Edith Lange January 15, 1979 Further placement of concete in safety related structures was halted at the Wolf Creek site on December 18, 1978.

The hold work agreement will remain in effect until KG&E has completed a review of the quality assurance program including review of procedures, implementation of the procedures and corrective action.

Prior to resuming the placement of concrete in safety related structures, KG&E corporate management will discuss with the Region IV staff, the status of their reviews and the corrective measures they have committed to, along with any significant findings.

In regard to your conments concerning placement of concrete during freezing weather, the applicable building codes that control concreting activities allow the placement of concrete during freezing weather, provided specific precautions are taken.

These precautions include provision of temporary shelters, and close control of concrete temper-atures during the batching, placing and curing activities in order to prevent freezing.
