SUBJECT:

Waterford SES Unit 3 l

Docket No. 50-382 j

Construction Permit No. CPPR-103 I

IE Inspection Report Na. 50-382/78-14

Dear Mr. Seidle:

LP&L's response to the item of noncompliance identified in the subject report is given below:

Failure to Establish Surveillance Inspection Program l

10CFR Part 50, Appendix B, Criterion X requires that a program for i

inspection of activities affecting quality shall be established to verify conformance with documented procedures for accomplishing safety related activities.

Tompkins-Beckwith Quality Assurance manual, paragraphs 11.4.1.1 and 11.4.7.1 specify that a Quality Control Program will be performed which will include recording of the QC surveillance inspection of welder's conformance to weld procedure requirements including gas flow, volts, amps, etc.

Contrary to the above:

Tompkins-Beckwith (T-B), who are involved in the welding of safety related piping, have not established a documented program for per-forming surveillance inspection of welder's conformance to weld procedure requirements.

This finding was confirmed during dis-cussion with T-B QA personnel.

This is an infraction.

Corrective Steps Taken and the Results Achieved:

Corrective action taken was in the form of incorporating the follow-ing paragraph into T-B Procedure TBP-26, Revision D, " Visual Inspec-tion":

6.1.14 Surveillance of welding parameters, i.e.,

amperage, voltage, gas flow rate, etc. will be performed daily on a random basis to verify conformance with the welding procedure being used.

Surveillance results will be documented on Form GP-723-12.

The above requirements were implemented on December 4, 1978.

2.

Corrective Steps Taken to Avoid Further Noncompliance:

Tompkins-Beckwith and Ebasco Quality Assurance will audit this area of activity during the month of February and on a periodic basis to verify and assure compliance by Tompkins-Beckwith.

3.

Date When Full Compliance Will Be Achieved:

Full compliance was achieved on December 4,1978.

Yours very truly, fbtWY
