Appendix A NOTICE OF VIOLATION

_ Calumet Testing Services, Inc.

License Na. 13-16347-01 Based on the inspection conducted on November 10, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. The following item is an infraction.

10 CFR 34.33(a) requires that pocket dosimeters be recharged at the start of each shift.

Contrary to this requirement, during the period February 14 through March 7,1980, pocket dosimeters worn by individuals involved in radio-graphy operations were not recharged at the start of each shift.

Speci-fically, your utilization logs for five dates during the above period showed initial dosimeter readings ranged from 40 to 112 milliroentgens.
