Appendix A NOTI _CE _OF VIOLATION Allied Inspection Services, Inc.
License No. 21-18428-01 As a result of the inspection conducted on November 19 and 21, 1980, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
License Condition No. 16 states, in part, the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in application dated February 3, 1979.
The licensen's Training Program,Section XI of their Operating and Emergency Procedures, which is included in the February 3, 1979 application, requires Radiographers and Assistants to receive periodic training and examination annually.
Contra ry to this requi rement, from June, 1979, to the dates of this inspection, the Radiographers have not received periodic training and examinations.
This is a Severity Level V violation (Supplement VII).

10 CFR 20.401(b) states, in part each licensee shall maintain records showing the results of surveys required by 10 CFR 20.205 (c).
Contrary to this requirement, from May, 1979 to the dates of this inspection, records of surveys of radiation levels external to packages received have not been maintained.
This is a Severity Level VI violatien (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within twenty-five days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or af firmation.
