Dear Mr. Seidle:
On October 23 while at the Wolf Creek Site, Mr. Oberg was advised of buckling in the floor liner plate of the refueling canal. The canal liner is not a Q listed item, however, on November 6 after removing a section of floor liner plate to investigate the problem, it was revealed that the top 4 to 5 inches of the underlying "Q" concrete was damaged.
This was also reported to Mr. Oberg.
The damage was the result of erroneously trying to pressure test the leak chases behind the liner plate.
It is our preliminary judgment that this problem, if left uncorrected, would not result in a significant safety hazard as addressed in Regulation 10CFR50.55(e). Our A/E is making an engineering evaluation of this matter and if this evaluation shows that damage to the floor of the refueling canal has safety significance, we will report further.
