Inspection on October 16-19, 1978 (99900288/78-01).

Areas Inspected: Action on eleven (11) previously identified inspection findings.

Implementation of 10 CFR 50, Appendix B criteria, and applicable codes and standards, including procurement document control; instructions, procedures, and drawings; and document control. The inspection involved twenty-three and one-half (23.5) inspector hours on site.

Results:

In the four (4) areas inspected, no unresolved items were iden-tified. The following deviations were identified:

Deviations: Action on Previous Inspection Findings - three (3) items had not been completed (Enclosure, Items A., B., and C. ); Document Control -

practices were not consistent with Criterion VI of Appendix B to 10 CFR 50 (Enclosure, Item D. ).

Action on Previous Insoection Findings 1.

(Closed) Deviation (Inspection Report flo. 77-01):

The inspector verified that Manuals delineating authority and duties had been issued for the following Departments: Material and Production Control; Manufacturing; Sales; Purchasing; and Engineering.

(Closed) Deviation (Inspection Report No. 77-01): The inspector verified that a statement had been added to the QC Manual indi-cating the Quality Control Department had sufficient authority to initiate, recomend, or provide solutions to quality problems.

3.

(Closed) Deviation (Inspection Report No. 77-01):

flumerous instances existed where serial numbers of gages used had not been recorded on order write-ups and final inspection reports; also, gage control provisions had not been placed on final inspection reports. The inspector verified that not all final inspection reports contained a gage control space; however, gage serial numbers had been recorded for all gages.

(Closed) Deviation (Inspection Report No. 77-01):

The inspector verified that performance of employees, engaged in bellows filling and automatic welding operations, is documented every six (6) months maximum.

It was also verified the procedure covering special processes had been deleted from the Quality Control Manual.

(0 pen) Deviation (Inspection Report tio. 77-01): Manufacturing and test procedures had not been issued for bellows filling and automatic welding operations, (See Enclosure, Item A.).

(Closed) Deviation (Inspection Report No. 77-01): The inspector verified that Quality Control Procedure No. 2 had been revised, deleting the requirement for completion of the reading column of the inspection record cards for measuring and test equipment.

(Closed) Deviation (Inspection Report No. 77-01): The inspector verified that Quality Control Procedure No. 2 had been revised to allow the use of labels or tags on measuring or test equipment that can't be identified by permanent methods.

The Quality Control Manual does not contain measures for internal corrective action, (See Enclosure, Item B.).

(Closed) Deviation (Inspection Report No. 77-01):

Audits had not been performed to satisfy Quality Control Procedures Nos.

5-2 and 5-3, (See Enclosure, Item C.).

(Closed) Deviation (Inspection Report No. 77-01): The inspector verified the Departmental Manuals required Manager's to audit their departments.

(Resolved) Unresolved Item (Inspection Report No. ~ -01): Pro-cedures have been written and assembled into the Engineering Manual.

Procurement Document Control 1.

Objectives The objectives of this area of the inspection were to verify that:

a.

Measures had been established to assure that applicable regulatory requirements, design bases, and other requirements necessary to assure adequate quality had been included or referenced in the documents for procurement of material, equipment, and services.

b.

To the extent necessary, procurement documents required suppliers to provide a quality assurance program consistent with the pertinent provisions of Appendix B to 10 CFR 50.

2.

Methods of Accomolishment The preceding objectives were accomplished by:

a.

Review of the following customer orders and attendant documents to verify that procurement document control had been invoked:

(1) Houston Lighting and Power Company Purchase Order No.

35-1197-4106, dated October 20, 1977.

- (2) South Carolina Electric and Gas Company Purchase Order No. SN-10225, dated January 13, 1977.

b.

Review of the following documents to verify that measures had been established to assure requirements necessary for adequate quality had been included or referenced in documents for procurement of material, equipment, and services:

(1) Section 17, dated May 31, 1978, of the Quality Control Manual.

(2) Purchasing Policy Manual, dated July 22, 1977.

c.

Review of the following purchase orders for major components:

(1) No. P16439, dated May 1, 1978.

(2) No. P15894, dated April 11, 1978.

(3) No. P16340, dated April 25, 1978.

(4) No. P16271, dated April 24, 1978.

(5) No. P14640, dated January 31, 1978.

(6) No. P15572, dated March 17, 1978.

Objectives The objectives of this area of the inspection were to verify that; a.

Activities affecting quality had been prescribed by docu-mented instructions, procedures, or drawings.

Tasks had been accomplished in accordance with those instruc-tions, procedures, or drawings.

c.

Quantitative or qualitative acceptance criteria had been included in the instructions, procedures, or drawings to indicate important activities had been satisfactorily accomplished.

Document Control 1.

Objectives The objectives of this area of the inspection were to veri fy that:

a.

Measures had been established to control the issuance of documents, such as instructions, procedures, and drawings, including changes thereto, which prescribed all activities affecting quality.

b.

Established measures:

(1) Assured that documents, including changes, are reviewed for adequacy and approved for release by authorized personnel, (2) Assured that documents, including changes, are distributed to and used at the location where the prescribed activity is performed.

c.

Changes to documents had been reviewed and approved by the same organizations that performed the original review and approval or another designated responsible organization.

2.

Methods of Accomplishment The preceding objectives were accomplished by:

The inspector met with the management representative denoted in paragraph A. at the conclusion of the inspection on October 19, 1978.

In addition, the inspector telephonically discussed the inspection with the addressee on October 23, 1978.

Management was requested to structure their response under headings of corrective action, preventive measures, and dates for each deviation.
