J Based on the inspection conducted on November 6 - December 1, 1978, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. Items 1 through 2 are infractions, and items 3 and 4 are deficiencies.
1.
10 CFR 20.101(b)(1) limits radiation dose to an individual to no more than 3 rems in a quarter.
Contrary to this requirement, a contractor employee received 3.07 rems during the third quarter of 1978.
2.
10 CFR 71.3 prohibits delivery of licensed materials to a carrier for transport except as authorized in a general license or specific license issued by the Commission, or as exempted in this part.
Contrary to this requirement, 11 cask shipments made between March 15 and July 27, 1978, were made without notifying the NRC prior to first use of the casks, including 10 made without copy of certificate of compliance on hand and 4 in secondary containers not permitted by the certificate of compliance.
3.
Technical Specification 3.12.5 requires notification of the NRC within 30 days when the annual projected release rate of radioactive materials in gaseous wastes, averaged over a calendar quarter exceeds twice the annual objectives.
Contrary to this requirement, the licensee made no notification after the third quarter release rate exceeded the annual objective release rate by a factor of 3.
4.
Technical Specification 4.ll.2.B requires daily gross alpha analysis of fire sump discharges.
Contrary to this requirement, the licensee has failed to make this analysis.

it appears that certain of your activities were not in accordance with acceptable practices, as noted below.
Contrary to recognized acceptable practices for air cleaning systems, the licensee operated the auxiliary building ventilation system for an unknown period between March and December 1978 with unsealed penetrations that undermined the test efficiency of the installed HEPA filters.
