APPENDIX A Weldex Corporation Industrial Radiographic Inspection 12223 Woodruff Avenue Downey, California 90241 License No. 04-04506-01 Notice of Violation Based upon the results of the NRC inspection conducted on April 23, 1980, one activity under your license appeared to be in noncompliance with NRC requirements as identified below:

Contrary to 20.408(b), reports summarizing individual exposures to radiation incurred during the period of employment by the licensee was not sent to the Director of Management and Program Analysis, U.S.N.R.C., Washington, D. C. 20555 for fo persons who left the li-censee's employ in March and August of 1979.
This is a deficiency.
