49-16670-01 i40TICE OF VIOLATION Based on the results of the NRC inspection conducted on December 7, 1978, certain activities appear to be in noncompliance with NRC regulations as indicated below:
1.
Contrary to 10 CFR 34.25(b), iridum-192 sealed source serial number 2252 was not leak tested within six month intervals during the period April 21, 1978 to December 7, 1978.
This is an infraction.
2.
Contrary to 10 CFR 20.102(b), personnel in a restricted area were allowed to receive exposure to radiation in excess of the limits specified in 10 CFR 20.101(a) during the period December 9, 1977 to December 7, 1978. Specifically, two personnel received 1.400 rem and 1.383 rem during the second quarter of 1978, and two personnel received 2.852 rem and 1.742 rem during the third quarter of 1978.
This is an infraction.
3.
Contrary to 10 CFR 20.405(a), written reports of overexposures were not submitted to the appropriate offices of the Nuclear Regulatory Commission, nor to the. individuals involved, when two personnel received 1.400 rem and 1.383 rem during the second quarter of 1978, and two personnel received 2.852 rem and 1.742 rem during the third quarter of 1978.
This is an infraction.
4.
Contrary to 10 CFR 19.13(d), copies of reports of exposure of an individual to radiation furnished to the Commission pursuant to 10 CFR 20.408, were not furnished to the concerned individual during the period December 9, 1977 to December 7, 1978.
This is a deficiency.
