APPENDIX A University of Arizona College of Engineering Tuscon, Arizona 85721 Docket No. 50-113 License No. R-52 Notice of Violation Based on the results of an NRC inspection conducted on January 9-11, 1979, it appears that one of your activities was not conducted in full compliance with conditions of your NRC Facility License No. R-52, as indicated below.
This item is a deficiency.
10 CFR 50.59(b) requires that a licensee's records include a written safety evaluation which provides the bases for the deter-mination that a change, test, or experiement does not involve an unreviewed safety question.
Contrary to the above requirement, the licensee records did not include a written safety evaluation to provide the bases for determining that a new thermal neutron irradiation facility, installed during 1978 in the reactor pool adjacent to the reactor graphite reflection assembly, did not invcive an unreviewed safety question.
