Pursuant to the Freedom of Information Act, 5 United States Code 522, as ammended, Energy Research and Information Foundation (ERIF) requests the following information:

1) Enclosed is a photocopy of a reportable occurrance which took place at the Duane Arnold Energy Center (DAEC) on May 19, 1978. We request all information the NRC has on this problem.
2) The date and the results of the last test prior to the test of the High pressure coolant Injection systemon May 19, 1978.
3) If the HPCI system was flawed on the previous test, the date and results of all previous tests of the HPCI system which determined that it did not work.
4) If the HPCI syst ,n was flawed on any test prior to the test of May 19, 1978, we request the a) dates and the actions the Nuclear' Reg-ulatory' Commission required towa Electric et al to correct the system, and b) the dates they were impimented.

Though the Freedom of Information Act does not require you to respond to the following points, would you please do so.

Does the HPCI system now work?

Is the NRC going to inspect the HPCI before operation starts up.

Does the NRC intend to fine Iowa Electric for the evnts which have occurred at the DAEC?

Was this issue brough up at any of the meetings held between Iowa Electric and the Nuclear Regulatory Commission which discussed the recirculation safe-end crack?

What is the current start-up for the DAEC?

Since ERIF intents to use this iriformation for public purposes, we request that you waive any fees incurred in this request. Please inform me of any fees before they are incurred.
