On January 31, 1979, during Mode 5 operations, S54-9 was discovered not functional in accordance with Technical Specification 3.7.15. The door was functional in that it was able to close the opening; however, the electric strike was out of adjustment and therefore the door could not make up with the door latch establishing a tight seal. This door is in the Unit No. 2 Emergency Switchgear and Relay Room. It shares, however, a common habitability with the Unit No. 1 Switchgear and Relay Room.

Probable Consequences of Occurrence:

The penetration fire barriers required by T.S. 3.7.15 are provided to assure continuous occupancy of the area during and after natural phenomena, fire and missiles as well as for all postulated accidents which may or may not release radioactivity to the environs. In this case, door S54-9 is used both as a pressure envelope boundary and a penetration fire barrier.

The health and safety of operating personnel and the general public were not endangered.

Cause The door latch had been recently replaced (see LER/R0 79-Oll/03L) and worked correctly. Subsequent to this, the strike worked itself loose causing it to shift position.

Immediate Corrective Action:

A fire watch was stationed to insure complete closure in order to maintain fire and pressure boundaries and the electric door strike was adjusted immediately.

Scheduled Corrective Action None required.

Action Take to Prevent Recurrence None required.
