Appendix A NOT, ICE OF VIOLATION License No. 12-17506-01 Nuclear Welding, Inc.
Based on the inspection conducted on August 6, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. This item is an infraction.
Condition 16 of your license states, in part, that the licensee shall use licensed material in accordance with statements, representations, and procedures contained in the letter dated April 11, 1977. An enclosure to this letter entitled " Instructions for use of Restricted Area and High Radiation Area Tables" provides procedures for estimating the physical dimensions of restricted areas.
Contrary to these instructions the inspector learned from licensee job survey sheets and interviews with personnel that these tables have ahrays been used incorrectly and in such a manner as to underestimate the required size of restricted and high radiation areas.
