1.0 Organization l.1 The final responsibility for the Quality Assurance Program for Part 71 Requirements rests with the Management of Quality Testing Inc.

Design and fabrication of radioactive material shipping packages shall not be conducted under this Quality Assurance Program.

The Quality Assurance Program shall be implemented by the Radiation Safety Officer (R30), reporting to the chief executive officer of Quality Testing Inc.

1.2 The RSO shall be responsible for meeting the requirements of Part

71. He shall be responsible also, for the overall administration of the program, training and certification, document control and auditing.

13 The RSO or the designated Assistant RSO shall be responsible for handling, storing, shipping, inspection, test, operating status and record keeping.

2.0 Quality Assur&nce Program 2.1. The Management of Quality Testing Inc, establishes and implements this Quality Assurance Program. Training for all QA functions, prior to engagement in these functions, is requirad according to written precedures. QA progrgs revisions will be made according to written procedures with management approval. The QA program will ensure that all defined QC procedures, engineering procedures,

and specific provisions of the package desigh approval are satisfied. The QA program will emphasize centrol of the characteristics of the package which are critical to safety.

2.2 The Radiation Safety Officer (RSO) shall assure that all radioactive caterial shipping packages are designed and manufactured under a Quality Assurance Program approved by the NRC for all packages designed or fabricated after July 1, IC78. This requirement will be met by maintaining on file, the certification to this effect, received from the manufacturer.

23 This QA program shall be used in conjunction with Quality Testing Inc. approved Radiation Safety & Centrol.vanual, which makes specific provisions for source receipt, handling, inspection, test, use, and disposal, and record keeping for these functions.

30 Docunent control 31 All docu=ents related to a specific shipment. will be controlled through the cse of written approved procedures. Documents shall be maintained en file with Quality Testing Inc. regarding sourc'e movement from receipt to final. disposition.

3.'2 The RSO shall insure that all QA functions are conducted in accordance with approved procedures, and that records are properly kept.

4.0 Handling, Storage and Shipping 4.1 Safety procedures as provided in the Quality Testing Radiation Safety and Control Manual, will be followed. Shipments will not be made unless all tests, certifications, and final inspections have been co=pleted. Refer to Part I section 3 of the Manual.

4.2 Radiographic personnel shall perform the handling, storage end shipping, with final responsiblity for co=pliance resting with the RSO.

50 Inspection, Test and Operating Status.

51 Inspection, test and operating status of packages for radioactive material will be indicated and controlled by tagging, labeling or log entry. Status of nonconforming parts or packages will likewise be noted by tagging or log entry.

52 Radiographic personnel shall perform the required inspections and tests in accordance with approved procedures. The RSO shall have final responsibility for assuring that these functions are performed and properly docunented.

6.0 Quality Assurance Records t

6.1 Records of package approvals, inspections, tests, logs and audit results, personnel training and qualifications and reccrds of shipments will be =aintained.

6.2 Records will be maintained in accordance with quality Testing Inc.

Radiation Safety and Control Manual. and will be availaole for review. The RSO shall keep shipping and receiving records on file with other pertinent Radiation Safety records.

70 Audits 7.1 Audits shall be perfor:ed, using established, approved procedures,

using checklists as provided for in the Radiation Safety and Centrol Manual, each activity being audited at least yearly. Audit results and reports will be maintained on file and deficient areas will be corrected. Audit results will be available for review.

72 Audits shall be performed only by persons having no responsibility in the activity they audit.
