Appendix A NOTICE OF VIOLATION Indiana University - Purdue License No. 13-02752-03 ' University at Indianapolis Based on the inspection conducted on February 1, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. Item 1 is an infraction and item 2 is a deficiency. 1. 10 CFR 20.207(a) requires that licensed materials, stored in an unrestricted area, be secured against unauthorized removal from the place of storage. 10 CFR 20.207(b) requires that materials not in storage must be under constant surveillance and immediate control of the licensee. As defined in 10 CFR 20.3(17), an unrestricted area is any area access to which is not controlled by the licensee for purposes of protection of individuals from exposures to radiation and radioactive materials. Contrary to this requirement, from statements of your employees a review of your radioactive receipt records, and a physical review of the radioactive receipt storage room, byproduct material is placed into a storage room at the University of Indiana - Purdue Medical Complex which is neither under constant surveillance nor under immediate control. 2. 10 CFR 20.203(e) requires that each area or room in which licensed material is used or stored and which contains any radioactive material (other than natural uranium or thorium) in an amount exceeding 10 times the quantity of such material specified in Appendix p of this part shall be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words: Caution Radioactive Material. Contrary to this requirement, the room in which radioactive material is stored upon receipt in the University of Indiana - Purdue Medical Complex was not posted with a sign bearing the radiation caution symbol and words: Caution Radioactive Material. __.}}
