Purpole.

To describe the quality assurance requirements which apply to the handling,. inspection, storage and ship =ent of the Tech Ops Model 650 Source Changer in compliance with article 71.51 of reference (a).

Backround. ; This command was registered by reference (b) to deliver licensed radioactive material to a carrier for transport in a Tech / Ops Model 650 Source Changer.

All personnel engaged in the operation (i.e. Packaging, Source Changing) of the Model 650 Source Changer are required to successfully complete the Nondestructive Testing of Metals Course, Class C administered at Service Schools Co= mand, San Diego, California.

An individual shall be designated as a Radioactive Material Shipping l,

Petty Officer by the Commanding Officer in accordance with article 5135 of reference (e). This assignment shall be designated in writing after the successful completion of a qualification examination approved by the Radio-logical Controls Officer and the Commanding Officer. The Radioactive Material Shipping Petty Officer shall be responsible for shipping Tech Ops Model 650 Source Changer in accordance with the requirements of reference (d) utilizing enclosure (2).

Document Control a.

The Radiographic Safety Officer shall ensure that certificates of compliance have been issued by the Nuclear Regulatory Commission for each shipping package utilized and that the cocmand is properly registered to use the package.

b.

The shipping package shall be controlled in accordance witn references (c) and (e).

Herlling, Storage, and Shipoing The handling, inspection, storage, and shipment of packages contain-a.

ing radioactive material shall be accomplished only by Command personnel for whom personnel dosimetry monitoring is provided.

Upon receipt of radioactive material, T-shed personnel shall notify Radiological Controls Personnel who shall perform and document radiation and contamination surveys on the shipping container. Additionally the material condition of the container shall be examined to determine suitability for The Radioactive Fbterial Shipping Petty Officer, Assistant Radiological reuse.

Controls Officer, or the Radiological Controls Officer shall verify the shipping container meets the applicable specifications of reference (d) for a Type B quantity of special form radioactive material. The results of the container material inspection and certification that the container meets the required specifications shall be documented on the initial radiological survey forms.

After movement to the Nuclear Support Facility, Repair 7 Division personnel shall be notified for pickup and final storage in the source vault.

Documentation shall be reviewed to determine that a leak test has been conducted within the past six months.

Transfer of sources from and to the shipping package shall be accomp-e.

Inspection, Test, and Operation natus The shipping package shall be prepared for return shipment of the a.

spent source in accordance with the criteria of references (d) and (e) utilizing the check list attached as enclosure (2).

Quality Assurance Records The Senior Radiographer shall maintain a record of the transfer of a a.

source to or from the shipping package insuring it is properly recorded in the Utilization Log.

b.

The Radioactive Material Shipping Petty Officer shall maintain a file of each shipment. Included as a minimum shall be the completed enclosure 2, a copy of the shipping papers, and a signed receipt from the organization the material was shipped to.

9.

Audits a.

The Radiographic Safety Officer shall audit the following areas semi-annually:

(1) Records maintained by the Senior Radiographer shall be inspected for completeness and accuracy.

(2)

Inspection and maintenance checklist required by reference (c) shall be reviewed to insure equipment is safe for use.

(3)

Shipping files maintained by the Shipping Petty Officer shall be inspected for completeness and accuracy required by paragraph 8b.

b.

The Quality Assurance Officer shall be informed of any deficiencies revealed by the audit.

Audit deficiencies will be documented on an Audit Deficiency / Discrepancy c.

Report Correction Form (QA Form-14) and forwarded to the Radiological Controls Officer who will determine corrective action to be taken within fourteen days.

The requisition number is determined as follows: The first set of numbers refers to the USS SPERRY and is always the sa=e(R04621).

The second set of numbers is the julian date. The last set of numbers is the designator for R-5 Division',' The R-5 RPPO ensures the requisition number is assigned by the Supply Department. This number is also known TCN for this shipment:

Radiological Controls Officer Co=manding Officer Step 18 has been properly completed.

Type one copy of the Radioactive Material Receipt form. Refer to step 3 for TCN.

This copy goes with the shipment inside the packing list envelope.

SIGNATURE 14.

Submit forms completed in Steps 11 and 13 to the Radiological Controls Officer for his approval and signature. Deliver the DD-ll49 form to the Supply Support Officer for typing of a DD Form 1348-1.

SIGNATURE 15.

Contact the Stores Officer or his Assistant to arrange for pickup of material at after brow on pier. Place the shipping label on the package, then place the rest of the documents in the packing list and place it on the container. Seal in accordance with CFR Title 49, Article 173.393.

Call the photographer to take pictures of the shipment. One picture should be taken of each angle so that all sides are clearly visible. All pictures must be in color. Staple the pictures to the shipping papers which will be kept by R-5 Division.
