Appendix A NOTICE OF VIOLATION St. Louis Testing Laboratories, Inc.
License No. 24-00188-02 Based on the inspection conducted on July 9 and July 11, 1980, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below.
Items 1 and 2 are infractions.
1.
10 CFR 20.201(b) requires that you make such surveys as may be necessary for you to comply with all sections of Part 20.
Contrary to this requirement on July 11, 1980 you failed to make such surveys as were necessary to assure conformance with 10 CFR 20.105(b), " permissible levels of radiation in unrestricted areas,"
a regulation that prohibits the licensec from creating radiation levels in unrestricted areas which could result in an individual receiving a dose in excess of two millirems in any one hour, or 100 millirems in any 7 consecutive days.
Specifically, radiography was performed in the permanent radio-graphy cell in such a fashion as to create 20 mr/hr radiation levels in unrestricted areas without surveys and exposure time evaluations necessary to assure compliance with 10 CFR 20.105.

10 CFR 34.42 requires that areas in which radiography is being performed shall be conspicuously posted as high radiation areas.
Contrary to the above on July 11, 1980, radiography was performed at the Wood River Shell Refinery, Wood River, Illinois, without the required high radiation area posting.
