License No. 43-11213-02 NOTICE OF VIOLATION Based on the results of the NRC investigation conducted on April 1-2, 1980, certain activities appear to be in noncompliance with NRC regulations and the conditions of your NRC License No. 43-11213-02 as indicated below:
1.
10 CFR 20.101(a), " Radiation dose standards for individuals in restricted areas," requires limitation of the whole body occupational exposure of an individual in a restricted area from radioactive material and other sources of radiation to 1.25 rems per calendar quarter, except as provided in paragraph (b) which allows a whole body exposure of 3 rems per calendar quarter provided that the individual's accumulated occupational dose to the whole body is determined on Form NRC-4, or on a clear and legible record containing all information required in that form.
Contrary to this requirement, the exposure of an individual was not limited to 3 rems during the first calendar quarter of 1980.
Specifically, a whole body dose of 12.78 rems was recorded for a radiographer during the first quarter of 1980..
This is a violation.

10 CFR 20.102(a) requires that the licensee shall require each individual, prior to first entry into the licensee's restricted area under such circumstances that the individual is likely to receive in any period of one calendar quarter an occupational dose in excess of 25 percent of the standards specified in 20.101(a), to disclose in a written, signed statement, either (1) that the individual had no prior occupational dose during the currout calendar quarter, or (2) the nature an2 amount of any occupational dose which the individual may have received during that specifically identified current calendar quarter from sources of radia-tion pcssessed or controlled by other persons.
Contrary to this requirement, such a signed statement was not obtained upon hire in January 1980 from an employee assigned duties associated with radiography.
This is an infraction.

10 CFR 34.29 requires that permanent radiographic installations having high radiation area entrance controls of the types described in 20.203(c)(2)(ii),
(2)(iii) or (4) shall also have a visible warning signal actuated by radiation and an audible warning signal which shall be actuated when an attempt is made to enter the installation while the source is exposed.

2 Contrary to this requirement, a permanent radiographic inscellation, in use on the day of the inspection and having high radiction.;rea entrance controls of the type described in 20.203(c)(4), was not equipped with such warning signals.
This is an infraction.
4.
License Condition 16 requires in part that licensed material be used in accordance with statements, representations, and procedures contained in the application dated August 31, 1978.
Contrary to this requirement, inspections of personnel required to be conducted at least once every 90 days in accordance with the licensee's Operating and Emergency Procedures, page OP-1, Item 2.c, were not con-ducted for one radiographer between August 15, 1979, and December 10, 1979.
This is an infraction.
5.
10 CFR 34.24 requires in part that a record shall be maintained of the results of each instrument calibration for two years after the date of calibration.
Contrary to this requirement, a calibration record for an Eberline model E-120G survey meter, serial number 200, calibrated on May 3, 1979, was not maintained for review on the day of the inspection.
This is a deficiency.
6.
10 CFR 34.42 requires that notwithstanding any provisions in 10 CFR 20.204(c), areas in which radiography is being performed shall be conspicuously posted as required by 10 CFR 20.203(b) and (c)(1).
Contrary to this requirement, a high radiation area incident to radio-graphy performed using an eleven (11) curie iridium-192 source on the day of the inspection was not posted.
This is a deficiency.
