Docket No. 99900505/79-03 NOTICE OF DEVIATION Based en the results of an NRC inspection conducted on August 7-10, 1979, it appeared that cer+'4 of your activities were not conducted in full co=p!"~s with NEC require =ents as indicated below:
A.

Enr,ineering i=plementing procedure No. E-70, dated May 20, 1979, (Notification of Proposed Design Changes In General Arrangement Draw-ings), establishes the requirements and instructions for processing design changes to General Arrangement Drawings by means of Ebasco Form 613. This form requires that reviewers sign and date their co=ments, that the Mechanical-Nuclear Design Supervisor sign and date each form, and when comments are attached by a reviewer the comments have to be resolved and so indicated on the form.
Contrary to the above, the following deviations from procedure E-70 were noted:
1.
On DCN 5 (Drawings HOV-7023 and M130006 - Reactor Containment Building), one of the revieaers required to com=ent marked his Form 613 "no comment," but did not sign or date the form as required by E-70.
2.
On DCN 75 (Drawing M120001 - Reactor Auxiliary Building):
a.
One of the reviewers required to comment atrached comments to Form 613, but did not sign and date the form to show when or if com=ents were resolved.
b.
The Mechanical-Nuclear Design Supervisor did not sign and
'date the form as required by E-70.
3.
On DCNs 55 and 57 (Drawing 140001 - Fuel Handling Building; Drawing M120001 - Reactor Auniliary Building), one of the reviewers used his initials in place of his signature.
B.
EBASCO Topical Report ETR-1001, "Ebasco Nuclear Quality Assurance Program Manual," Section QA-I-6, Revision 0 (Quality Assurance Records) 1422 539 7911'290 3 statas in part (paragraph 4.7), "When QA Records are corrected or supple =ented they shall be reviewed or approved by the organization tha: criginated the records. All corrections and supplements shall bear dates and au-J2cri:ed signatures."

Con :ary to the above, Quality Program Procedure No. 5 entitled
" Deviating Ebasco Project - Related Quality Assurance Programs frem the Ebasco Nuclear Quality Assurance Program Manual" has not been issued, nor has any other procedure been issued to provide control of such deviations to assure that safety and/or quality will not be sacr'ificed. Project-related QA Program deviations have, in fact, been issued, although no procedure exists to provide control by prescribing approval requirements.
