In addition to the above technical and supervisory personnel, the IE

inspector held discussions with various maintenance, operations, technical

support and administrative members of the licensee's staff.

Licensee Actions Taken in Response to IE Bulletin 79-02, Revision 1

a.

Review of Testing and Tnspection Procedure

The IE inspector reviewed special procedure SP-Anchor-1, " Inspection /

Testing of System Support Base Plates Using Concrete Expansion Anchor

Bolts."

This procedure establishes the inspecticu and testing require-

ments of seismic Category I system support base plates that use concrete

expansion anchor bolts.

The licensee initiated his testing and sampling program based on the

suggested procedure of IE Bulletin 79-02 of testing one bolt per base

plate. These base plates were selected from a representative sample

of all seismic Category I systems.

This representative sample in-

cluded the Feedwater, Raw Water, Safety Injection, and Auxiliary

Coolant systems. A testing and sampling program is currently in

progress to include those seismic Category I systems not included in

the representative sample.

This statistical sample is to provide a

95 per cent confidence level that less than 5 per cent defective

anchors have been installed in any one seismic Category I system on

a system by system basis.

The testing program has been developed for Phillips Red-Head Shell

and Stud type anchors. The testing portion of the procedure verifies

that anchor bolt preload is equal to or greater than bolt design load.

The concrete expansion anchors are tested to the excess proof load

if the plug depth for shell type anchors and if the embedment depth

for stud type anchors are not within the procedural acceptance

limits. However, acceptance criteria for these anchors after the

test load is applied is as defined in the procedure for the proof

load requirements.

The IE inspector reviewed procedural requirements for verification

by inspection that the concrete anchor bolts are properly installed

and are of the specified size and type. Among the parameters to be

included in the licensee's inspection are:

All existing expansion anchors which are to be replaced will be

replaced with Hilti Wedge-type anchors. All anchors which are to

be used in a new location will likewise be "ilti Wedge-type anchors.

At the time of this inspection, qualification tests were being per-

formed at the site on the Hilti Wedge-type anchors to establish

proper installation torque requirements for these concrete anchors.

All of the concrete expansion anchors which do not meet the pro-

cedural acceptance criteria will be replaced. All of the base

plates with those concrete expansion anchors which do not meet the

safety factor of five criteria established by IE Bulletin 79-02,

Revision 1 by the analysis method will be repaired immediately if

the analysis indicates a safety factor less than two.

If the anal-

ysis indicates that there is a safety factor greater than two, but

less than five, a pull test to the design load of all of the con-

crete anchor bolts in that particular base plate will be conducted.

Repairs to that base plate are based on the results of the pull

test.

b.

Work Observation

The IE inspector observed activities relative to field inspection

and testing procedures for Support No. FPS-16 and Support No. CHVS-25.

Ultrasonic testing methods were observed for Support No. FPS-16 and

these methods were discussed with the licensee personnel performing

the ultrasonic testing. Calibration data for UT Instrument No.

NDT-131-089, Torque Wrench T-20, and Load Cell No. HWID-07154A were

found to be current at the time of this inspection.

Field inspection activities were observed and completed inspection

activities were reviewed from existing documentation. The IE

inspector confirmed that a QC verification had been incorporated

into the testing and inspection procedures and activities. All

activities observed and all documentation reviewed were in accordance

with SP-Anchor-1.

The IE inspector met with members of the licensce's staff at the

conclusion of the inspection.

The scope of the inspection and the

findings were discussed.
