Division of Compliance, licadquarters INQUIRY MEMOPAliDUM IIEPROPOLITAN EDISON CCMPAN't, (TIIREE MILE ISLAND), 289/68-A CONCRETE VOIDS - TENDON GALLEY Mr. Vernon Steubner, Resident Engineer, Three Mile Island (Met-Ed) 1968, to report telephoned Region I at 10:00 a.m. on December 16, voids in the concrete of the tendon galley of the containment The pertinent facts obtained from Mr. Steubner are as building.

The concrete for the reactor base is essentially complete.

When the forms were removed from two sections of the tendon 2.

The voids were caused by scrap wood and mud.
