This item is an infraction.
10 CFR 20.201(b), " Surveys," requires that eacn licensee make or cause to be made such surveys as may be necessary to comply with regulatory requirements specified in 10 CFR Part 20.
Contrary to the above, during the period March 23 - April 1,1978, individuals worked inside Steam Generators A and B where localized ganra radiation fields of up to 45 R/hr existed near tube sheet surfaces providing the potential for hand and foot doses in excess of 25 percent of the applicable extremity value specified in 10 CFR 20.101(a), and the survey made to determine whether extremity cersonnel monitoring was required, pursuant to 10 CFR 20.202(a)(1),
was not adequate in that it failed to reveal the need for such menitoring as was subsequently discovered during a resurvey con-ducted April 4, 1978.
