cn December 3, 1971 by Metropolitan Edisen Cc=pany concerning Three Mile Island IIuclear Statica Unit ?!o.1 concerns unexpected problems in the south 180 of the ring girder discovered upon thorough visual inspection of that area. These problens vere discussed in the interis report filed with the AEC by Metropolitan Edison Cc=pany en May 31, 1972.
Of principle concern is the portion of the Ring Girder from elevation th3 ft - 1/h in. to elevation L50 ft - 6 in. clockwise fres Azi=uth 106 to 288 en the outboard side.
This area is referred to in this addendu= as the " South 180 ".
This addendus provides additicnal infer:stien en Lifts 3 and k as shown in Figure A-5 of the above report ec= piled frem the investigatien as described in the interis report. Included herein is a cer;1ete description of the extent of the problems, corrective actions and precedures , and the analysis of the resultant (repaired) structure.

2.00 RIiG GIRDER CCIISTRUCTIC:I The south 180 was not accessible for visual inspection when
. the " Report on Contain=ent Building Ring Girder Ccnstructica and Repair," dated Dece=ber 1,1971, was filed. Since that ti=e an investigation of this area was =ade. See Section 3 0 of this addendus.
The report on the north 180 of the ring girder was directed toward the i= proper eensolidation of the concrete. When paragraph 2.03.h of the Dece=ber 1,1971 report on the south 180 was written concerning the south 180 it also was directed toward the proper consolidation of concrete. Investigation has shown that consolidation of the concrete was greatly i= proved over the north 1800; hcvever, the construction joints were deter =ined to be the questienable area.
i The new constructica joint locations as shov= cn Figure A-5 of the Dece=ber 1,1971 report was sub=itted to and approved by GAI on March 31, 1971.
Lift 1 and Lift 2 renained the sa=e for the entire 360 and vere relatively shallow lifts with good accessibility and visibility.
Rebar had not been placed for Lift 3 thus providing good access to the lifts below. Lift 1 was placed in October,1970, and Lift 2 was placed in Nove=ber, 1970.
Keyvays were placed in all lifts and ta= ped into the fresh concrete. Due to the density and location of the rebar, the keyways were not continuous as indicated on drawings. The joints were prepared by various =ethods such as green cutting, raking, or wire brushing. The face was cleaned by vashing and bleving with an air jet. Epoxy was applied by spraying.
Regarding the reported apparent ncn-cenfor=ance with Appendix 3 to 10 CFR 50, Criterien V, we have ec= piled the following docu=ents which substantiate the fact that the construction joints in questien did contain concrete keys and the location (in elevatien) did not confor= to prescribed drawing require =ents. The as-built surface variation is docu=ented on UE&C drawing 9459-FS-335.

Peference docu=ent, 1 above, detailing construction joint locations and keys vere approved by GAI Engineering and distributed (UELC Quality Centrol Group included) prior to cennencing verk in the centain=ent ring beam area, south half.
This =cets Criterien V require =ents.
Reference doct=ents , 2 above, indicate satisfactorily installed and prepared concrete keys in the contain=ent ring bens, south half.
Reference drawing, 3 abcve, shows the deviation of the construction joints (in elevation) frem the location as shows en the sketches in enclosure 1 above.
The criginal plan called for the concrete Lift 3 to be placed to elevation L53 ft - 6 in. Weed linerferns, tenden tru= plates, and rebar had therefere been installed to that elevation. When the new lift schedule was approved as =uch rebar as possible was renoved to provide limited access for placing and vibrating the concrete. This rebar was replaced prior to placing the next lift.
Lift 3 was placed in two segnents, the first on April 5, 1971, and the secend on April 6, 1971. The finished elevatien of these placenents was difficult to =aintain because of the folleving reasons:
(a) congestion of tendon tru= plates , form liners , and rebar; (b) the portien of the construction joint visible when for=s vere re=oved at a later date could not be seen during placenent; (c) the a= cunt of concrete at each release of the bucket or pu=p was difficult to centrol due te allevable variations of slu=p ; and , .
(d) the congestien of rebar for ed da=s which did not allev the concrete to =ove laterally as readily as it vould under less severe conditions.
Due to a ec=bination of the abcve conditions , the constructica joint between lifts 3 and k at the outside face nov shows a variation of 2 1 foot frcs the required elevatien of kk6 ft 9.in.

Although the li=ited access provided by the removal of the rebar vorhed well for the place ent and vibratien of the 20ncrete ,
cleaning of the joint was still difficult. The outboard area of the joint between the tendon duct and the for:s was extrer.ely congested and the bearing plate ferned a ledge under which dirt could becc=e entrapped. This conditi:n was existent for the joints between Lifts 2 and 3, and between Lifts 3 and h.
Lit. k vas placed in ene segnent on April 15, 1971. Access for cleaning of the joint between Lifts 3 and k was similar to that between Lifts 2 and 3 The proble=s which existed in maintaining the finished elevation of Lift 3 vere partially present for Lift k.
Lift 5 was placed in one ses=ent on April 22, 1971. The placement was basically the sane as a 3 ft - 0 in. floor slab. Rebar con-gestion was not a probles. Visual inspection of the joint between Lifts k and 5 indicated no problems.

, supporting the north half ring girder repair effort, was extended arour.d the scuth half portien of the ring girder for the purpose of repairing app rently tinor concrete voids. Upon elese visual ext ination and =anual preting it beca:e evident that the surface voids vere =cre than superficial. On April 26, 1972, a Field Change #2 to CCP-9, "Contain=ent Ring Girder Repair" (construction precedure originally approved for north 180 area of girder area) was issued in order to initiate exploratory steps for removal of surface debris inclusiens and honeycc b, and core drilling on the south 1300 . ,In additien, Field Change #3 to CCP-9 vas issued en May 11, 1972, to, include re=cval of certtin tenden tearing plates in the south ISO in order to verify soundness of cenerete behind ,
the plate area. Frequent meetings of GPU, GAI, and UE&C repre-sentatives vere held te evaluate results of explcration steps as cc:pleted, and to issue directions for subsequent exploratory steps. During the course of exploration, the degree of joint centact at the heri: ental construction joints (belev the icver layer and telev the upper layer of deze tru plates) was questiened.
A prcgra= cf exploratory cere drilling through the construction joint was begun in an effort to evaluate the joint and to plan repairs as necessary.
An attachsent to thl= allend = includes a set (c3 sheets) of core plottings and Icg analysis for each core drilled; a prepared
" Report of Core Analysis" prepared ,by Pittsburgh Testing Lateratcry; and a stretch-out drawing (Drawing 1) illustrating the horicental construction joints, the core locations in relatic: to tenden tru= plate locations, and preli=inary void / repair infor=atien.

h.00 RI':G GI?l:ER REpA!R - I'; SOUTH 180 k.01 CONCRETE RD'07AL Concrete excavation was initiated in the areas of surface debris inclusions, and surface honeyec=b vas re=oved to visually sound concrete. For the purpose of orientation, refer to Figure A-5, as contained in the Dece=ber 1, 1971, " Report en Contain=ent Building Ring Girder Construction and Repair." In general Figure A-5 illustrates the lift segnents e= ployed for place =ent of concrete in the south girder area. At certain upper and lover layer do=e tru= plates , the bearing plate was or vill be re=oved, and excavation of honeyec= bed concrete to a sound face vill be perfor=ed. The re= oval of tearing plate is perfor=ed so as to reduce the possibility of danage to rebar and to reduce the gross volu=e of sound concrete that =ust be re=cved for access to the affected area.
Where heri: ental construction joints are censidered unacceptable as deter =ined by core boring, exa=ination and engineering evaluation, the affected joint will be expcsed and cleaned to predeter=ined depths by = cans of chipping tools. It vill be necessary to open a cavity at the joint with the outside height of the cavity increasing or decreasing in direct propertien to the depth of the cut.

h.02 REINFORCING STEEL REMOVAL A'TD VERIFICATION OF SOUND CONCRETE Reinforcing steel re= oval and verification of sound concrete vill be perfor=ed in accordance with the outline given in sub-section h.01.2 and k.01.3 of the above referenced Dece=ber 1,1971, Report ,
and addendu=.
h.03 REPAIR A'iD REPLACDENT Cnce it has been deter =ined that only scu=d concrete re=ai=s and all da= aged e=beeded cc=ponents have been re=oved, the repair and replace =ent of all affected ec=penents vill begin in a logical sequence to be determined in advance of constructics.
k.03.1 Reeair of Tenden conduit and Reelace=ent of Reinferein: steel, Strain Gares and Gr.,und Cable Repair of tendon conduit, replace =ent of reinforcing steel, reinstallation of strain gages, and connection of ercu=d cable vil' be perfor=ed in accordance with the outline given in sub-sections 4.02.1, L.02.0, sua k.C2.3 af the above referenced Decenter 1, 1971, report and aidendu=.

h.03.2 Retla:enent of Tru= plate Bearin Plates Bearing plates were re=oved by an internal annular cut through the tru= plate tube a shcrt distance behind the rear cf the bearing plate, chipping concrete adjacent to the bearing plate edges, and finally lifting the plate away fro the cenerete.
Bearing plates vill be replaced by initially aligning the plate and its stub tube to the enbedded = sting tube by use of an internal =andril, securing the plate in positien with an external jig, re=oving the =andril, and velding the tube ends together at the annular cut.
h.03 3 concrete Placement Following concrete re= oval, verification of sound concrete, and repair of all enbedded ec=pc;ents, the place =ent of concrete in all cavities shall be perfer=ed in accordance with the outline of sub-section h.02.h of the Dece=ber 1,1971, report and addendu=,
except for references to Seg=ent Types I, II, and III areas which are not applicable. The areas of repair in the south 1600 ring girder are illustrated en attached Dravings no.1, lA,13, ic, and 1D.
Guidelines as to sequence of final concrete re= oval and replace =ent, per rpecific areas vithin the ecuth 1600, shall te deter =ined in accordance with evaluatien and final deter =ination by the Engineers group. ,
4.03.k Ouality Assurance and control Quality assurance and centrol shall be perfor=ed in accordance with sub-section h.02 5 of the Dece=ber 1,1971, report and the addendu=.
h.0h SUZZARY The repair of the Three Mile Island I.ing Girder scuth 180 , A:i=uth 108 te Azi=uth 238 , is li=ited to ;he area at the heri: ental construction joint below the lover layer of do=e tendons and at the hori: ental construction jo'.nt i==ediately above the upper layer of dome tendons.
The areas to be repaired were affected by 11=ited visual and physical accessibility, ineffectual joint preparatien and cleaning of previcusly placed cenerete surfaces, and ineffectual ecnsolidation of concrete in localized arens due to limited accessibility.

In addition to the unsound concrete re=oved, a considerable a=ount of sound concrete vill be recoved to provide increased assurcnce that only sound concrete re=ains and to provide excavations of the best possible configuration for accepting replace:ent cencrete.
Damaged' tendon conduit will be repaired in accordance with approved procedures in such a =anner that conduit vill =eet all require =ents of the original specification.
Da= aged reinforcing vill be replaced and spliced by lapping, Cadwelding, or full penetration butt velding techniques.
Reinforcing vill be as criginally detailed.
Strain gages a=d electrical ground connections vid be checked and replaced as required and rechecked.
Concrete replace =ent vill be =ade using the sa=e concrete =ix as originally used. Tne closure pour of each seg=ent vill include a pouring speut to provide hydraulic head to further ensure drilling of all recesses. These speuts vill be re=oved and the surface rubbed to conform to the required cutline.
All repair work vill be inspected by the UE&C Field Quality Centrol Group and ^211y docu=ented.

5.00 ST.S.Ess ANALYSIS The two hori: ental construction joints at elevatiens kh3'-0" and hho'-9" have been investigated for the transfer of stresses across the joint considering representative load co=binations. The two joints are shcan on Figure 1 which also shows the direction of the existing shear rein-
" forcing (rebar) with respect to the construction joint.
Shear

_The primary concern of this investigation has been the transfer of shear
- . across the constructien joints. The folleving conservative assu=ptions have been =ade: - ~
___.c-1. There is no tensile capacity in the concrete across the joint, due to.the presence of bond-breaking =edia. -
] .- ._ _- - 2. IntLnate contact of the concrete surfaces exist at the joints enabling full transfer of co=pression across the joints.
3 The existing sh-a- --4nforcing across the joints has been counted only when the direction of the shear is positive as shown in Figure 1 engaging the shear rebars in tensien. Cnly the ec=penent of the shear bar along the direction of the joint is considered.
- k. There is aggregate-interlock type shear capacity across the joint because the old concrete obtained caly a rough finish. It is, therefore, j=stified to ecunt en a friction coefficient of at least u = 1.0, i.e. , one used for concrete cast against hardened concrete.
5 No keyvay is assu=ed for transferring the shear. A load factor of 1.k has been used for the ncn-accident lead ec=binations - Nor=al Operation, Test Ccndition, Initial Prestress. The analysis =ethod deviates frc= conventional reinforced concrete theory by neglecting any shear capacity of the cenerete at the construction joints, except where such is developed in shear friction by the presence of a ec=pression force acting perpendicular to the joints. This is a conservative assu=ptics and it ec= plies with the field conditiens observed. Substantial shear reinforce =ent (stirrups) intersects the construction
. joints at angles of 20 and 26 degrees respectively. While in conventicnal reinforced concrete theory such shear reinfcrce=ent is censidered effective, as long as its angle with the cross-sectienal plane is s= aller than k5 degrees, the above analysis neglects the effectiveness of this steel for conditions where the radial shear, Q, acting along the ecnstruction joint vould engage this steel in cenpressien rather than in tensicn. '

In other vords , in co=pliance with the "zero-bond" assu=ption, the potential shear failure plane is assu=ed at the construction joint instead of the co==caly assu=ed h5 degree principal tensile stress failure plane. Tnis _ assu=ptien is correct in view of the field conditions observed.

- 3. Shear force on constructica joiats:
The shear and nornal forces on the constructica joints are shown in Table 1. Figure 1 shovs the sign conventica of the shear and nor=al forces acting on the joint surface as indicated. Positive shear, Q, engages the shear reinforcing in tension. Thus , for the accident-lcad condition, the shear rebar capacity, Vult, is ~- - greater than the positive shear force, Q, across the jo17t for both the upper and lover joints. For the other three (3) representative lead con 31:ations, the
- -- - required coefficients of friction p to transnit the shear across the joint is less than one (1) for all three (3) ce=binations at -both the upper and lever construction joints (see Table 2) .
The results shown in Table 2 are obtained using the conventional friction equation: Q = uN Where, Q = shear resistance force N = nor- conpressive force p = static coefficient friction Q and N are obtained frc= the analysis and p is then calculated using the above equation. Variance in Construction Joint Elevation The variance in the horisental constructica joint elevation for the joints at approxicate elevatiens th3' and 440'-9" in the South half of the ring beam shown in Drawing i has been evaluated. In some places, the joint varied upward on a s=ceth transition through the spiral reinforcing behind the bearing plate. The anchorage :ene reinforcing has been censervatively designed to account for transverse tensile stresses in the bursting :ene. Therefbre, although the construction joint passes through the anchorage
- ene, the anchorage :ene vill functicn as intended. The everall effect of the varied elevation construction joint on the irng beam behavior has been considered along with the investigatien of shear stress transfer across the joints. The variation frem the intended elevation is a s:ceth transition as shown on Drawing 1 and vill not affect the transfer of stresses in the ring beas.

Su==ary The two (2) horizontal constructica joints at elevations 4h3' and hh6'-9" in the South half of the Reacter Building ring bea= have been found to be capable of transferring the shear across the joint for the four (h) representative load cases. For the accident load case, the shear rebars are engaged in tension due to the positive direction of the shear; the rebars are sufficient to transi'r the shear solely by friction in less than one for the accident load conditica. For the other three,(3) load cases, the shear rebars are also engaged in tension at both joints. However, the rebars are not sufficient to account for the shear. Therefore, for these three (3) 1 cad cases , it has been shown that the required coefficients of friction to transfer the shear across both upper and le' joints is less than one. O

Discussion of Results The quality of the concrete rr> presenting the tuo lifts were sound except uhere specifically chewn in the core logs. This evaluatica specifically covers the condition of the two construction joints. Terminology used in the core logs which do not indicate a sericus condition are dilated as follows:
- 1. Slight Porosity 6: typical voids - this is typical porosity that may be found in sound and consolidated concrete.
- 2. Forous due to weod or other fragments - this condition reduces the cor. tact surface a negligible amount since it is only slightly greater than porosity normally encountered.
- 3. Powder film - this serves as a bond breaker but is of very slight thickness and should enable a transfer of compressive loads through the joint without significant strains. ,
The core lor.s are given as an appendix to this report which are a duplicate of the core logs indicated on the individual cross sectional drawingr. of the ceres drawn by U.E. & C. The evaluation was made from the drawings tinich depicted the radial penetration of the cores. (Attached) Evaluation The follcuing evaluation is given by lengths described by A zones indicated for the Joint at Elevation 443' (1cucr) and the B : opes indicated for joints at elevation 446'9" (upper). Ecroscope e:cmination was made to supplement the core data and it was fouW, thrit the core infor:rai-ion was core reliable for the evaluatien.
. The evaluation describes the general penetration of defective joints. The actual excavation n.tst be ade to tae depth where a sound joint is visible in order to provide for localized arcas.
