APPENDIX A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on July 20-23, 26, 29 and 30, 1976, it c,' pears that certain of your activities were not conducted in full compliance with conditions of your NRC License No.

DPR-50 as indicated below.

These items are Infractions.

A.

Technical Specification 6.5.1.6.e states in part that the POBC shall be responsible for:

"(1) Review of...any violations of these Technical Specifications or Operating License DPR-50... and (2) review of violations of applicable federal statutes, codes, regulations and internal station procedures and instructions having nucicar safety significance."

Contrary to the above, nu=erous violations of the type described above identified in twenty-eight Station Nonconformance Reports and five QA Audit Reports covering the period of March 1 through June 30, 1976 were not reviewed by the PORC.

Prior to the completion of the inspection, the actions taken to

correct this Item of Noncompliance and to prevent recurrence were reviewed by the inspector and found acceptable.

No response to this item is required.

B.

Technical Specification Section 6.11 states that radiation protec-tion procedures shall be prepared consistent with the requirements of 10 CFR 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure.

1.

Procedure HPP 1613, " Radiation Work Permits", developed pursuant to the above, requires all personnel to be authorized by and logged in on Radiation Work Permits, (RWP) prior to entries into areas posted with specified signs.

I Contrary to the above, the following were allowed to occur:

i a.

An individual who was not authorized by a RWP was observed in a posted and roped-off Radiation Area on the 281 ft, elevation of the Auxiliary Building at 2:50 p.m. on July 20, 1976.

Three individuals who had failed to log their reentry under RWP No. 8852 were observed in a posted and roped-off Radioactive Materials Area at 2:57 p.m. on July 20, 1976.

c.

Seventcen RWP records for. work completed between July 1, 1975, and Jura 30, 1976, were found to lack required entries.

I 2.

Procedure HPP 1776, " Sampling of Unmonitored Release Paths",

developed pursuant to Technical Specification 6.11 specifies eight samples monthly, including one of the sewage system.

Contrary to the above, none of the eight specified samples were obtained during February, 1976, and only or.e monthly l

sample was obtained of the sewage system during the twelve j

month period July 1, 1975 through June 30, 1976.

Other Infractions and Deficiencies identified through your internal audit program and which were corrected, or corrective action was initiated, are set out in the inspection report. No additional information is needed for these items at this time.
