This telegram is to confirm the conversation between Mr. J.L. Seelinger, Unit 2 Superintendent Technical Support, and Mr. D. Haverkamp, Region I NRC at 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> on October 19, 1978.
A situation considered reportable under technical specification 6.9.1.3.5 was discovered at 1000 hcurs on October 19, 1978, when it was determined tnat the limiting condition for operation (LCO), action b, for T.S. 3.1.6.2 was not invoked when surveillance procedure, 2301-301, data obtained at 1935 on 10-16-78 showed an Unidentified Leakage greater than 1 gpm (2.6 gpm actual unidentified leakage).
This event cccurred due to personnel errors in interpreting both the LCO and surveillance performance requirements of T.S. 3.4.6.2.
Unidentified leakage was reduced to T.S. limits at 0735 on 10-18-73.
Further cetails and corrective action will be discussed in the followup report.
