ORGANIZATION:
The final responsibility for the Quality Assurance Program for Part 71 requirements rests with Worthington Pump Corporation, USA.
Design and fabrication of radioactive material shipping packages shall not be conducted under this Quality Assurance Program.
The Quality Assurance Program is implemented using the following organizational chart.

The Radiation Safety Officer is the responsible individual for the Part.71 Quality Assurance requirements.
The Radiation Safety Officer is responsible for overall administration of the program, training and certification, document control, and. auditing.
The radiographers are responsible for handling, storing, shipping, inspection, test, operating status and record keep-ing.
2.
QUALITY ASSURANCE PROGRAM:
The Management of Worthington Pump Corporation, USA estab-lishes and implements this Quality Assurance Program.
Training for all Quality Assurance functions, prior to engage-ment in these functions, is required according to written operating procedures.
Quality Assurance program revisions will be made according to written procedures with management approval.
The Quality Assurance Program will ensure that all defined O.A.
procedures, engineering procedures and specific provisions of the package design approval are satisfied.
The Quality Assurance Program will emphasize control of the characteristics of the package which are critical to safety.
The Radiation Safety Officer shall assure that all radioactive material shipping packages are designed and manufactured under a Quality Assurance Program approved by the Nuclear Regulatory Commission for all packages designed or fabricated after July 1, 1978..isuasoma,or w eatsiNoros atua Nc

DOCUMENT CONTROL:
All documents related to a specific shipping package will be controlled through the use of written procedures.
All document changes will be performed according to written procedures, approved by management.
Procedure used in obtaining and returning radioactive materials:
A)
Initiate purchase requisition for management approval.
B)
Copy of formal purchase order and number assigned, returned to N.D.E. Department.
C)
Isotope received at WPC.
Receipt of delivery signed by -RSO or radiographer and Isotope is surveyed to insure shipping container is in good condition and not damaged.
D)
Isotope is stored in exposure room in a safe area until it is transferred to the projector.
E)
Authorized Licensed Representative notified to transfer source.
F)
Source transferred to proper projector by Authorized Representative and decay chart removed from shipping container and transferred to NRC Record Log Book.
G)
New sourre surveyed in projector and initial readings recorded on decay chart.
Date, time and Authorized Representative's initials.
H)
Enter in radiographers daily log sheet.
I)
Enter in quartly log inventory record book.
J)
Proper tags, labels and identification of shipping container to be returned to manufacturer for dis-posal.
K)
Source in shipping container under direct control and responsibility of NDE Department, Radiographer, ar.d/or R.S.O.
until it is shipped from WPC.
L)
Shippers certificate for radioactive materials properly filled out and signed by radiographer or R.S.O.
M)
Shipping manifest signed and dated by carrier.
N)

All documents per this procedure are located in the NDE Department, NRC Record Log Book and Radiographer's Daily Log Book.
The Radiation Safety Officer shall insure that all Q.A.
functions are conducted in accordance with the latest applicable changes to these documents.
4.
HANDLING STORAGE AND SHIPPING:
Written safety procedures concerning the handling, storage and shipping of packages for certain special form radio-active material will be followed.
Shipments will not be made unless all tests, certifications, acceptances, and final inspections, acceptances and final inspections have been comp-leted.
Work instractions will be provided for handling, storage and shipping operation.
Radiographic personnel shall perform the critical handling, storage and shipping operations.
5.
INSPECTION, TEST AND OPERATING STATUS:
Inspection, test and operating status of packages for certain special form radioactive material will be indicated and con-trolled by written procedures.
Status will be indicated by tag, label marking and log book entry.
RSO will be notified of status of nonconforming parts or packages.
Reports and records will be kept for all nonconforming packages and supplier notified in writing.
Radiographic personnel shall perform the regulatory required inspections and tests in accordance with written procedures.
The Radiation Safety Officer shall ensure that these functions are performed.
6.
QUALITY ASSURANCE RECORDS:
Records of package approvals (including references and drawings) inspection, tests, operating logs, audit personnel training and qualifications and records of shipments will be maintained in the NRC Record Log Book.
Description of equipment and written procedures will also be maintained.
The required records will be on file in the NDE Office and will be identifiable and re-trievable.

AUDITS:
Established schedules of audits of the Quality Assurance Program will be performed using written check lists.
Results of audits will be maintained,and reported to management.
Audit reports will be evaluated and deficient areas corrected.
Audit reports will be dependent on the safety significance of the activity being audited, but each activity will be audited at least once each year.
Audit reports will be maintained as part of the Quality Assurance records.
Member of the Audit Team shall have no respon-sibility in the activity being audited.
