In connection with our review, we need the information identified in the enclosure to this letter.
Please submit seven copies of your response to the enclosed request for additional information within 30 days following receipt of this letter. Also, please delete Attachment II f rom the application. While there must be supporting uritten procedures to implement your QA program, these procedures and forms should not be submitted as part of your QA program plan.

Request for Additional Information APR 131979 The scope of the audit program should be expanded to include such things as personnel training, personnel qualifications, record keeping, etc.
Provide a statement in part 7 that these types of functions will be audited at least annually.
