Organization The final responsibility for the Quality Assurance (QA) Program for Part 71 Require ents rests with ABCO Perforstors, Inc.

Cesign and Fabrication shall not be conducted under this OA Program.

The QA Program is implemented as shown on the attached organiz3 tion ch a rt.

The Radiation Safety Officer is responsible for overall administration of the program, training and certification, document control, and auditing.

The Radiographers are respocsible for handlina, storing, shipping, inspection, test and operatirg status and record keecing.

nuality Assurance Drocram The management of ABCO Perforators, Inc.

(Lteensee Name) establishes and implerents this QA Program. Training, pricr to engagement, fer all QA functions is required according to written procedures. QA Program revisiens will be made accordino to written procedures with management approval. The OA Program will ensure that all defined CA procedures, engineering procedures, and specific provisions of the package design approval are satisfied. The CA Program will emphasize centrol of the characteristics of the package which are critical to safety.

The Radiation Safety Officer shall assure that all radioactive raterial shippina packages are designed and ranufactured unda-a OA Program approved by Nuclear Regalatory Commissien for all packanes desiered or fabricated af ter July 1,1973-This requiro ent can be satisfied by receiving a certification to thil effect frcm the ranuf acturer.

Docu ent Control All documents related to a specific shipoing nackace will be controlled through the use of written procedures. All dccument chances will be performed according to written procedu es approved by ranage ent.

The Radiation Safety Officer shall instre that all CA functions are conducted in accordance with the latest acplicable chances to these docurents.

Findling Storace and Shiccin, Written safety procedures ccccerning t"e handling, storage and shipping of packages 'or certain s:ecial form radicactive raterial will be fol!cwed. Shipments will not be made unless all tests, certifications, acceptances, and final inspections have been co cleted. Work instruct-ions will be provided for hand 1irg, storace, and shicping operations.

Radiography personnel shall per'orn the critical handling, storace and shippina Operations.

Inscection, Test and 0 erating Status Inscection, test and Operating status of packages for certain special form radioactive material will be indicated and controlled by written procedures. Status will be indicated by tag, label, marking or log entry. Status.3' nonconforming parts or packages will be positively

Radiogrichy personnel shall perform tre regulatory required inspections and tests in accordance with written cr0cedures. The Radiation Safety Officer shall ensure that these functions are perfor ed.

Descriptions of equi; rent and written procedures will also te maintained.

These records will be raintained in accordance with written procedures.

The records will be identifiable and retrievable. A list of these records, with their storage locations, will be maintsired by the Radiation Safety Officer. '

Audits Established schedules of audits of the CA Program will be perfor ed using written check lists. Results of audits will be raintained and reported to manage-ent. Audit re;crts will be evaluated and deficient areas corrected.

The audits will be dependent c7 the safety significance of the activity being audited, but each activity 4111 be audited at least cnce per year.

Audit re:: orts wi'l te aintair.ed is part of the quality assurance records.

Members of the audit team -hall have no resconsibility in the activity being audited.
