1.0 INTRODUCTION

1.1 Purpose This manual was prepared for the purpose of furnishing employees of Industrial Testing Laboratory Services Corp. with practical information on an efficient Quality Assurance Program for special form radioactive material.

Design and fabrication shall not be conducted under this QA Program.

1.2 Scope All personnel engaged in operations involving special form radioactive material must comply with the provision of this manual.

1.3 Responsibility A.

Radiation Safety Officer The Radiation Safety Officer shall assure that all radioactive mitterial shipping packages are designed and manr.factured under a QA Program approved by the Nuclear Regulatory Commission for all packages designed or fabricated after July 1, 1978.

This requirement can be satis-fied by receiving a certification to this effect from the manufacturer.

B.

Training prior to performing any QA function is required according to written procedures with management approval.

The QA Program will ensure that all defined QA Procedures, Engineering Procedures and specific provisions of the package design approval are satisfied.

The QA Program will emphasize control of the characteristics of the package which are cricical to safety.

2.0 ORGANIZATION 2.1 Authority and Duties for Quality Control A.

The Radiation Safety Officer will have complete responsibility for the maintenance of the program dealing with special form radioactive material.

The Radiation Jafety Officer will report directly to the Vice President-Chief Engineer.

All records, reports and procedures will be maintained by the Radiation Safety Officer or other trained personnel designated by him.

B.

Industrial Testing Laboratory will be responsible for the Quality Assurance Program but will delegate those portions which are accomplished by the manufacturer of the equipnent. A certification of com-pliance will be requested from the manu-facturer on an annual basis by the Radiation Safety Officer.

3.0 DOCUMENT CONTROL A.

All documents, instructions, procedures and drawings will be reviewe'd by the Radiation Safety Officer before being issued to ITLS personnel.

No changes will be made unless approved by the Radiation Safety Officer.

B.

All QA functions will be conducted in accor-dance with the latest applicable documents in force for that function.

4.0 HANDLING, STORAGE AND SHIPPING A.

The established safety restrictions concerning the handling, storage and shipping of packages for certain special form radioactive material will be followed.

B.

Shipments will not be made unless all required tests, certificatior.s, acceptances and inspec-tions have been coupleted.

C.

No personnel will handle, place in storage or ship any special form radioactive material without the proper instructions on that operation.

D.

Only the radiation safety officer or a radio-grapher delegated by the RSO in writing will perform the above QA functions.

5.0 INSPECTION TEST AND OPERATING STATUS A.

Inspection test and operating status of packages for certain special form radioactive material will be indicated and controlled by established procedures.

B.

Status of a package will be indicated by tag, label, marking or log entry as determined by the Radiation Safety Officer.

C.

Any non-conforming parts or packages will be status reported only by the Radiation Safety Officer.

D.

Only the Radiation Safety Officer or a radio-grapher delegated in writing by the RSO will perform the above QA funcions.

E.

It is the radiation Safety Officer's responsi-bility to ensure these functions are performed correctly.

6.0 QUALITY ASSURANCE RECORDS A.

Records will be maintained and include operating log (utilization record), inspections, tests as received from equipment manufacturer and audit results.

B.

Records will be maintained on personnel quali-fications, descriptions of procedures, and equipment as stated in our Operating and Emergency Procedures Manual.

C.

Records will be maintained as to be identifiable and retrievable.

D.

A list of required records ar.d their storage locations will be maintained by the Radiation Safety Officer.

7.0 AUDITS A.

An audit of the Quality Cor crol Program will be made by the president of the corporation on an annual basis.

The extent of this audit will cover each activity under the direct control of Industrial Testing Laboratory.

B.

The results of audits will be maintained by the Radiation Safety Officer and reported to the Vice President-Chief Engineer within 30 days of the audit.

C.

Audit reports will be evaluated and deficient area corrected by the Radiation Safety Officer.

D.

Audit reports will be maintained as part of the Quality Control records.
