FEB 011979 APPENDIX A NOTICE OF VIOLATION U. S. Nuclear, Inc.
License No. SNM-1315 Based on the results of the NRC inspection conducted on January 8-9, 1979, it appears that certain of your requirements were not conducted in full compliance with h3C requirements as indicated below.
These items have been categorized as described in our correspondence to you dated December 31, 1974.
10 CFR 20.408 and 10 CFR 19.13(d) require that results of bioassay information required by license condition be furnished to the NRC and to individuals within 90 days of termination of employment.
The licensee was required by Condition 8 of the license to conduct a bioassay program in accordance with Regulatory Guide 8.11 Contrary to the above:
1.
The reports to the hTC dated March 7 and May 2, 1977, included the results of only the last urine sample and did not include results of lung counts.
2.
The reports to eight individuals dated April 29, 1977, did not include all urine results and included only the last lung count.
This is a deficiency.
