APPENDIX A NOTICE OF VIOLATION United Nuclear Corporation Docket No.70-820 This refers to the inspection conducted by representatives of the Region I (Philadelphia) office at United Nuclear Corporation facility, Wood River Junction, Rhode Island of activities authorized by NRC License No.

SNM-777 During this inspection, conducted January 9-16, 1979, the following apparent items of noncompliance were identified.

1.

Condition 9 of the facility license, which incorporates Section 205, " Process Control" of the licensee's approved license applica-tion, dated October 13, 1975, requires, in part, in paragraph 205.1, " General," that supervision at all levels assure themselves that all pertinent regulations, controls and procedures relative to nuclear criticality safety are followed by supervision and all operating personnel.

The posted criticality safety limit for the product scale in the process area is one product container or 350 grams of U-235, and the posted criticality safety limit for a desk in the nondestructive assay area is one product container or 350 grams of U-235.

Contrary to the above, on January 9, 1979:

a.

Two product containers containing U-235 in excess of the posted limits were observed on the product scale; and, b.

Two product bottles containing U-235 in excess of the posted limits were observed on the desk in the nondestructive assay area.

This is an infraction.

Condition 9 of the facility license, which incorporates Section 205, " Process Control" of the licensee's approved license applica-tion, dated October 13, 1975, requires, in part, in paragraph 205.1, " General," that supervision at all levels assure themselves that all pertinent regulations, controls and procedures relative to radiological safety are followed by supervision and all operating personnel.

S0P I-F, Revision II, dated August 7, 1976, " Health Physics,"

states in Section A.3:

"The hands will be washed and monitored prior to obtaining cigarettes, coffee, or lunch or leaving the plant."

S0P I-H, dated August 7, 1976, " Contamination Control," states in attachment "Non-Assigned Personnel," that personnel leaving the contamination control area will " monitor hands, then all personal clothing" and Section 3.B states:

" Nuclear and Industrial Safety will assure that radiation monitors are provided, calibrated and operating correctly at the contamination area exit."

Contrary to the above:

a.

On January 10 and 11,1979, personnel were observed smoking in the locker room on the restricted side of the change line.

The personnel did not wash or monitor for contamination prior to smoking.

b.

On January 9,1979, a licensee employee was observed moni-toring with an inoperable monitor.

c.

On January 10, 1979, a licensee. employee was observed exiting the contaminated area and not monitoring.

This is an infraction.

3.

Condition 9 of the facility license, which incorporates Section 307, " Marking and Labeling" of the 1icensee's approved license application, dated Octover 13, 1975, requires, in part, in 307.1,

" Criticality Limits," that signs listing approved nuclear criti-cality safety limits be posted so that information is readily discernible to employees.

Contrary to the above, on January 11, 1979, three bottles of high er.riched uranium and one bottle of low enriched uranium were observed stored on the bookcase above the desk in the foreman's office.

There were no posted criticality safety limits for this area.

This is a deficiency.
