Telephone Inquiry Concerning Survey Questionnaire This refers to a telephone inquiry by Ms. M. Campbell of this office with yourself on January 2,1979.

This inquiry was an examination of activities authorized by License No.

SNM-ll30, as they relate to the infonnation documented in your response to the NRC survey questionnaire.

From this discussion, it is our understanding:

1.

that, contrary to your response to question 12 of the question-naire, there have been no radiation exposures to individuals in excess of the limits specified in 10 CFR Part 20, Section 20.101.

2.

that your radioactive source is kept in secure storage and used approximately once a month.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter will be placed in the Public Document Rocm.

No reply to this letter is required; however, should you have any questions concerning this inquiry, we will be pleased to discuss them with you.
