ggg Appendix A NOTICE OF VIOLATION Board of Trustees, University License No. 12-15780-01 of Illinois Peoria School of Medicine Based on the inspection conducted on February 7, 1979, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Item 1 is a deficiency and item 2 is an infraction.

1.

10 CFR 20.401(a) requires that radiation exposures of individuals be maintained on Form UhC-5 or on clear and legible records con-taining all the inf ormation required by Form NRC-5.

Contrary to the above, you have maintained incomplete exposure records, in that they did not contain all the information required by Form NRC-5.

Specifically, your exposure records did not con-tain the social security numbers or dates of birth for persons working with licensed byproduct material.

Condition 15 of your license requires that licensed material be possessed and used in accordance with statements, representations and procedures contained in application dated October 23, 1975, letter dated December 30, 1975, and Radiation Safety Handbook dated March, 1974, a.

Contrary to Item 11 of application dated October 23, 1975

(" Methods, Frequency, and Standards used in Calibrating Instruments"), which requires the annual calibration of your " Cutie Pie" Ion Chamber and ICN Geiger Counter, it was learned through statements of licensee representatives and the absence of records that no instrument calibrations were performed during the years 1976 and 1977.

b.

Contrary to Section II, Part A.14, of the above referenced Radiation Safety Handbook, it was learned through state-ments of licensee representatives and the ab;ence of records, that the requirement for quarterly meetings of the Radiation Protection Committee is not being met.

(Only one Committee meeting report could be produced for the period December 7, 1973 to February 7, 1979).
