Occurrence Date:

January 4,

1979 l

Identification of Occurrence:

i A review of safeguard cable re sting identified eight cabic< which were imprope rly i

designated as non-safeguard cabtes.

l Condition Prior to Occur _rence3 i

Unit 2 operating at fu ll powe r.

Unit 3 operating at approximately 9 M power.

Apparent Cause of Occurrence:

Leproper designation of eight cables associated with the RCIC and IITCT controls on both units during the design phase.

Analysis of Occurrence:

During the design phase of the Fesch Bottosi units, an error in designating j

the DC power supply cable and AC cable associated with the static inverters i

in the RCIC and llPCI systews was ande.

This was identified during a design 1

review of cable routing initiated as a result of an investigation with respect l

to fire protection.

Specifically, the eight c@les involved were originally j

considered non-safeguard and were rmited accordingly.

The enbles associated

]

wkth the PCIC should have been identified as safegua rd cr;ht. 5 and routed in a 7A divialen.

The cables as?ociated with the 3(PCI hould have been designated

=

as safeguard and routed in the IB division.

- Routed in all non-safeguard trays HPCI Routed in att IB trays.

Not labeled as a ZB cable.

AC Cable All cable routings for HPCI and RCIC are in non-safeguard trays.

Unit 3 However, the AC cables of the IIPCI and the RCIC occupy the same non-safeguard tray in one-vertical section.

The safety significance of this event is sonsidered minimal.

All cables involved are located in the cable spreading room or control room.

All cable trays involved are seismic qualified.

Fire detection devices have been installed in the cable spreading area since original installation.

Corrective Action:

tamediate corrective action involved the stationing of a fire watch on a full time basis in the area where these cables are improperly routed.

New cable-routings are being designed.

New cable, property designated and rmted, will be installed in the near future.

The present schedule calls for completing work on Unit 2 on January 4th.

Work on Unit 3 will be completed prior to the end of the Unit 3 outage which is scheduled to start on January 5th. The fire-watch will be maintained until new cables are installed.

Previous Failures:

None similar.
