s License No. 26-16871-01 Notice of Violation Based on the results of the fiRC inspection conducted on November 14 and 15, 1978, certain activities appear to be in noncompliance with NRC regulations and the conditions of your fiRC License No. 26-16871-01 as indicated below:
1.
Contrary to License Condition 17, which requires that licensed material be used in accordance with statements, representations and procedures contained in application dated November 3,1975 and letter dated March 18,1976, a Ludlum, flodel 3, SN: 3424, survey meter was not calibrated at six month intervals during the period !1 arch 30 to October 18, 1978.
This is an infraction.
2.
Contrary to 10 CFR 30.51, records of receipt of a sealed source containing 125 millicuries of cesium-137, 3M flodel 4F68, were not maintained on November 14 or 15,1978.
This is a deficiency.
