Results: Of the six areas inspected, no items of noncompliance were identified.

Materials, Facilities and Equipment Materials As of the close of business on December 31, 1979, the licensee ceased all operations in the Chicago, IL area. No material has been received by the licensee since this date.

As of the date of this inspection, the licensee had executed Form NRC-314, " Certificate of Disposition of Materials" and forwarded it to NRC HQ Washington, D.C.

A copy of this form is attached as Ex-hibit A.

The inspector verified that no material remained.

Facilities The licensed facilities (third floor, Rm. 301, 210 S. Desplaines St., Chicago, IL) had been cleaned, awaiting final check and subse-quent removal of equipment.

Equipment The licensee had almost all of their equipment consolidated in the

" Visitor's Waiting Area" of their facilities awaiting final disposi-tion (see Exhibit B, sketch of licensee's facilities, attached).

Licensee plans to relocate this equipment to one or more of its other remaining, licensed, locations.

No items of noncompliance were identified.

Exposure Controls - External The inspector reviewed personnel exposure history for the last quarter of 1979. During this period, one individual's ring b:dge showed a total of 41,250 millirem and the body badge shawed a tote 3 of 610 millirem. The licensee had performed an evaluation of this extremity exposure and determined that the individual had removed his gloves and deposited them in a waste drum. The ring badge had come off his finger in the glove and the individual did not realized this fact.

The ring was subsequently located in the waste drum. The licensee calculated and assigned a total exposure of 3920 millirems to the individual's extremeties. This value was submitted to the processor and the appropriate exposure was assigned to the inolvidual.

No items of noncompliance were identified.

Surveys On April 14, 1980, the licensee performed a comprehensive survey of their facilities for both fixed and loose activity. The results of these surveys are presented in the copy of licensee's letter dated April 18, 1980, which documented information concerning their dis-continuation of this operation (Exhibit C, attached). The licensee determined that no fixed radiation levels were in excess of back-ground levels and that no loose radioactive contamination was detectabic. The RIII inspector performed confirmatory surveys of the licensee's facilities on April 16, 1980. Fixed radiation levels were surveyed with both a bete gamma, end window, GM survey instrument (E-500B, No. NRC 000640, cali Tted 1/26/80) and a gamma scintillation survey instrument (Thyac II,

. NRC-000706, calibrated 3/15/80).

Loose activity levels were also measured by the inspector. The results of these surveys are presented in Section 6, " Confirmatory Measurements".

No items of noncompliance were identified.

Radioactive Effluent and Waste Disposal The licensee stated in their letter dated April 18, 1980 (Exhibit C),

that no materials were dumped or placed into the sanitary sewer. This was confirmed by the RIII inspector who performed appropriate surveys on lavatory and sink drain lines.

The licensee had collected all remaining dry waste and transferred it to their facilities in Pittsburgh, PA during February,1980. The package weighed 169 lbs. and contained selenium-75, cobalt-57 and iodine-125 waste. The measured 8 mR/hr on contact and 0.4 mR/hr at three feet. Wipes taken prior to departure from the Chicago, IL facilities and after arrival at the Pittsburgh, PA facilities both indicated no loose contamination from the surface of the package.

The package was transported by licensee personnel in one of their vehicles used for distribution of materials.

No 'tems of noncompliance were identified.

Confirmatory Measurements As stated in Section 4 of this report, an RIII inspector performed confirmatory surveys at the licensee's facilities on April 16, 1980.

No fixed, beta-gamma contamination levels in excess of background levels (0.01 to 0.02 mR/hr) were identified.

In addition to the fixed activity surveys, the RIII inspector also took several wipe surveys to determine the presence of loose radio-dClivity Contamination. These wipes were subsequently measured at the RIII facility; none of the wipes indicated activity levels in excess of Minimum Detectable Activity (MDA) levels which were 2

a calculated to be 47 dpm/100 cm (21 picocuries/100 cm ).

This level is much less than that specified for release of material and equipment for unrestricted use. The results of these wipe samples are presented in Exhibit D attached.

No items of noncompliance were identified.

8.

Exit Interview The RIII inspector informed those individuals present (denoted by as-terisks in Section 1) of his findings, with regard to the fixed radioactive contamination, and that if any of the wipe samples indicated loose, radioactive contamination, they would be advised accordingly. As stated in paragraph 7 above, subsequent analyses of the wipe samples showed no activity.
