APPENDIX A NG7CE OF VIOLATION Rad Services, Inc.

Leurel, Maryland 20810 NRC General License Based on the results of an NRC inspection conducted on December 12, 1979, it appears that certain of your activities were not conducted in full compliance with NRC regulations as indicated below:

10 CFR 150.20 states that persons who have a specific license in an Agree-ment state are granted a NRC General License to conduct the same activity in a non-Agreement state provided certain specified requirements are met.

These requirements include that the general licensee is subject to the provisions of the specific license issued by their Agreement state.

Condition 14 of your Agreement State License No. MD27-012-01 requires that licensed materials be possessed and used in accordance with Statements, representations and procedures contained in your application dated April 12, 1976. This application includes your form entitled " Radioactive Waste Disposal Service Work Order" which requires that certain specified radia-tion surveys be performed when a package of radioactive waste is received by one of your company representatives.

One of the surveys required by this form entitled " Radioactive Waste Disposai Service Work Order" is an external radiation survey of packages.

Contrary to this requirement, a package was received from an NRC licensee in Wilmington, Delaware on December 7, 1979 which was not properly surveyed or recorded in that external radiation levels greater than background were misrecorded on your Radioactive Waste Disposal Service Work Order form as background levels of radiation.

2.

One of the surveys required by this form is a contamination survey on the outside of packages.

Contrary to this requirement, packages were received from an NRC licensee in Pittsburgh, Pennsylvania on September 18, 1979 and on October 16, 1979 and a contamination survey was not performed on those packages.

One of the surveys required by this form is a survey of the external radiation levels at the driver's seat of the vehicle used for trans-port of packages.

Contrary to this requirement, a package was received from an NRC licensee in Pittsburgh, Pennsylvania on September 20, 1979 and no radiation survey was performed at the driver's seat of the vehicle used for transport of this package.

These are infractions.
