b 8005070 Appendix A NOTICE OF VIOLATION Isotope Measurements Laboratories, License No. 12-13568-01 Incorporated Based on the inspection conducted on February 15, 1980, it appears that certain of your activities were in noncompliance with NRC requirements as noted below.
Item 1 is an infraction, and items 2 through 3 are deficiencies.
1.
Paragraph 10 CFR 20.201(b), Title 10 of the Code of Federal Regulations, Part 20, states, each licensee shall make or cause to be made such surveys (evaluations) as may be necessary for him to comply with the regulations in Part 20.
Contrary to this requirement, as of the date of this inspection, the evaluation of an apparent whole body overexposure to an individual during the fourth quarter of 1979, had not been completed to verify compliance with 10 CFR 20.101(a).

Paragraph 10 CFR 30.51(a), Title 10 of the Code of Federal Regulations, Part 30, states, in part, each person who receives byproduct material pursuant to a license issued pursuant to the regulations in this part, shall keep records showing the receipt, transfer and disposal of such byproduct material.
Contrary to this i airement, as of the date of this inspection, records of the rect.pt and transfer were not available of nominal 200 microcurie vials of cesium-137 received during November 1978, for use with dose calibrators.

Paragraph 10 CFR 35.14(e)(2), Title 10 of the Code of Federal Regulations, Part 35, states, in part, any licensee who possesses sealed sources as calibration or reference sources shall cause each to be tested for leakage and/or contamination at intervals not to exceed six months. No leak tests are required when the sealed source is stored and not being used; such sources shall be tested for leakage prior to any use or transfer unless they have been leak tested within six months prior to the date of use or transfer. The leak test shall be capable of detecting the presence of 0.005 microcuries of radio-active material. Records of leak test results shall be kept in units of microcuries and maintained for inspection by the Commission.
Contrary to this. requirement, as of the date of this inspection, no leak test records were available for nominal 200 microcurie vials of cesium-137 transferred for use as calibration and/or reference sources in dose calibrators.
