A Appendix A NOTICE OF VIOLATION Rockf ord College License No. 12-08009-02 Based on the inspection conducteu on February 22, 1980, it appears that certain of your activities were in noncompliance with NRC require-ments as noted below.

Item I is an infraction, and items 2 and 3 are deficiencies.

1.

Condition 16 of your license requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in your application dated November 21, 1973.

Fbterial submitted under item 11 of this above application states contamination survey instruments will be calibrated at the beginning of each college term.

Contrary to the above, the survey instruments in use in koom 102 (the physics laboratory) and Room 322B (the biology laboratory) in the Starr Science Building have not been calibrated since June 4, 1970.

Condition ISB of yoc license requires leak test records of sealed sources be maintained for inspection by the Commission.

Contrary to the above, on February 22, 1960, no leak test records for the period of September 10, 1976 to the date of the inspection were available for review by the inspector.

10 CFR 19.ll(a) and (b) require current copies of Part 19, Part 20, license conditions, documents incorporated into the license, license amendments and operating procedures be posted, cr a notice describing these documents and where they may be examined, be posted.

10 CFR 19.11(c) requires Form NRC-3 " Notice to Employees" be postea.

10 CFR 19.ll(d) requires these documents and notice appear in a suf ficient number of places to pe rmit individuals engaged in licensed activities to observe them.

Contrary to this requirement, on the day of the inspection, February 22, 1960, neither the documents or the notice were posted in either the biology or physics laboratory.
