PUGET SOU'iD NAVAL SHIPYARD QUALITY ASSURANCE PROGRAM PLAN FCR SHIPPING PACKAGES 1.
ORGANIZATION:
a.
The Conmander, Puget Sound Naval Shipyard, has the overall responsibility for the Quality Assurance (QA), rogram for 10CFR71 re-quirements regarding shipping packages. The QA program is irrplemented under the organization as shown in the Attachment A charts.
b.
The Radiological Control Ofcer (the Director, Radiation Health, Code 105.5) is responsible for the overall administration of the Ship-yard shipping container QA program to the extent of assuring performance of the actions specified in paragraphs 1.c through 1.e below. He is also specifically responsible for providing container inspection and associated records for cnntainer user: other than Code 135, surveys of all incoming and oucgoing shipments, and instructions for action in any cases of container defects, damage, or non-compliance, c.
The Special Progrant Branch, St.pply Department (Code 513) is responsible for providirg container shipping and receipt precedures, inspections of incoming and outgoing containers, and assos f ated records,
i and for reporting to Code 105.5 any container damage, defect, or non-comoliance detected.
d.
The Non-Destructive Test Division, Code 135, is resconsible 5
for surveying and for inspecting all radiographic source shicaing con-tainers for damage or defects, and for preparing and maintaining the associated records of such inspections.
Code 135 is also resconsible for the prcoer handling, use, anc storage of such containers while the containers are in their custody.
The Quality Assurance Office, Engineering and Analysis Division e.
t (Code 136), is responsible for auditing all aspects of the QA program for shipping packages at least annually, according to the written pro-cedures for this program.
2.
QUALITY ASSURANCE PROGPAM:
The QA prcgram established and implemented by the Shipyard a.
ensures that all 10CFR71 requirements, procedures, and package design approval provisions are satisifed and emphasizes control of package characteristics critical to safety.

The Director, Radiation Health ensures tnat all radioactive material shipping packages used by the Shipyard and designeo or fab-ricated af ter the effective date of the QA program were designed and manufactured under an NRC-accroved QA program, iils is accceclisned by obtaining certific3 tion from the package ~:anufacturer.
Design and fabrication of packages is not conducted under tne Shipyard QA prcgr3r.

Each Department / Shop having responsibilities in this OA program assures by appropriate training, evaluation, and certification that all OA functions are performed by qualified personnel.
d.
Any revisions of this Shipyard QA program or procedures are l
made according to the established Shipyard Instructions regarding such changes.
Management review and approval are required according to these Instructions.
3.
DOCUMENT CONTROL:
a.
All documnts related to specific shipping packages and changes thereto are controlled according to the established Shipyard document control Instruction (s), with the required management review and approval.
The Director, Radiation Health is responsible for assuring that all QA functions are performed according to the latest applicable document changes.
4 HANDLING, STORAGE, AND SHIPPING:
a.
The handling, storage, and shipping of certain special form radioactive material packages are done in accordance with written p rocedures.
Shipments are nade only when all tests, cartifications, and final acceptances as applicable have been completed. Code 135 personnel perforn the critical operations of handling, storage, and preparation for shipping of radiography sources. Code 105.5 directs preparation of other sources for shipment. The shipments are made under the direction of Code 513.
5.
INSPECTION, TEST, AND OPERATING STATUS:
a.
The inspection, test, and operating status for certain special fonn radioactive mterial packages are specified and controlled oy written procedures, and are indicated by tag, label, marking log entry or individual log sneet. These tests and inspections are performed by qualified Code 135 radiograchy personnel for radiography sourcas in accordance with the aoplicable procedures.
The Director, Radiaticr Health ensures that these functicns are perfonned and is responsible for performing the functicns for packages other than those under Code 135 cognizance.
Status of nonconforming packages is also maintained according to written prccedures.
6.
QUALITY ASSURANCE RECORCS:
a.
Quality assurance records are maintained in accordance with written procedures, and are identifiable and retrievable. These re-cords include package approvals, inspections, tests, coerating logs, audit results and responses, personnel training, and shiprent records.
These records are stored and retained for fcur years by the activities preparing them as scecified in the written procedures, and are avail-able for inspection.

AUDITS:
1.
Audits are perfomed at least annually of each Shop / Code in-volved in the QA program, using written check lists; and audit results are recorded and reported to management. Any deficiencies are required to be corrected by the responsible Shop / Code. Audit reports and re-sponse/ corrective action reports are maintained as part of the QA records.
Audit team members are selected from personnel who have no responsibilities in the activities being audited.

Note 1:
The Director, Radiation Health is designated the Shipyard Radiological Control Of ficer (the form NRC-313 " Radiation Protection Officer") for matters under NRC cognizance. The Director, Radiation Health has direct acccss to the Shipyard Coninander for matters concerning radiation health and NRC Byproduct Material License, and has additional duty to the Ilead, Branch Clinic and the Director, Occupational Health Service for matters concerning radiation health.
Note 2: The Radiological Control Branch (Non-Nuclear) provides professional and technical services for all ionizing radiation matters under the Shipyard NRC License control, including NRC sicensing and licensed sources (such as radiographic and Radiac equipment); and industrial x-ray machines; x-ray I
spectrographs; radium sources; and radiographer and Radiac operator training.
Note 3: Provides personnel dosinntry services, and radiation exposure recording and reporting services.
