99900039/79-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on July 9-13, 1979, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR 50 states in part that, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriata to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings Deviations from these requirements are as follows:
A.
Paragraph 10.2.2.C of the ASME accepted QA manual states in part tha t', "At the completion of a performance qualification the Senior Welding Engineer prepares a performance qualification record that includes as a minimum the name of the welder or welding operator, the identification number assigned to the welder by the Senior Welding Engineer, a brief description of the welding process including the required variables, and the date and results of the qualification test.
This record is verified and signed by the Senior Welding Engineer, Contrary to the above requirements, upon completion of performance qualifications, performance qualification records were not prepared for two (2) welders.
B.
Weld procedure No. 389-500-A72, Revision 2 requires that welding be performed with the amperage between 125 to 150 amps.
Contrary to the above requirements, it was observed that Weld A of a pipe support identified as job number 389, requiring the use of Weld Procedure No. 389-500-A72, Revision 2, was being welded with 100 amps.
