Very truly yours,

ATTACINENT i
1.
Item 1 - ANO - 1 Technical Specification 3.3.5 states in part, " Prior to initiating maintenance on any component of a train in any system, the redundant component of that system shall be demonstrated to be operable within 24 hours2.777778e-4 days
0.00667 hours
3.968254e-5 weeks
9.132e-6 months
prior to the maintenance".
Contrary to the above, the Low Pressure Safety Injection Pump P-34B was shown to be hold carded for preventative maintenance on June 8, 1978 by Job Order No. 2141.
The redundant train component P-34A was not shown to be operabic within the previous 24 hours2.777778e-4 days
0.00667 hours
3.968254e-5 weeks
9.132e-6 months
of June 8, 1978.
Response to Item 1 This incident was discussed with the Shift Supervisor that was on duty at the time of the infraction.
On February 15, 1979, a memo (Ops. 34) was issued to all Unit 1 Shif t Supervisors directing them to review the Technical Specification requirements in regard to operability tests of redundant components before removing equip-ment from service.
This is the first occurrence of failure to test the redundant train component prior to performing maintenance.
Based on this fact, the above corrective action should preclude any repetition of this type of incident.
2.
Item 2 - ANO - 1,2 Technical Specification 6.8.1 requires that, " Written procedures shall be established, impicmented and maintained covering..
the applicabic procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978".
Contrary to the above, a "Neckly Report of Overduc Maintenanco" is not being prepared each week as specified in Section 3.3.3.5 of Plant Procedure 1005.02.
Response to Item 2 This non-compliance was caused by not having an individual assigned the specific responsibility of coordinating the preventive maintenance program.
l As of February 15, 1979, we have assigned the specific responsibilities to an individual who reports to the Planning and Scheduling Supervisor.
The weekly reports are now being maintained in accordance with the procedure.
Plant Procedure 1005.02 is in the final stages of a revision to upgrade the program and will be approved by h! arch 30, 1979.
Based on the above action we feel that the plant will be in full compliance on blarch 30, 1979.
