APPENDIX A NOTICE OF VIOLATION University of Virginia License Nos.: R-66 Based on the results of the NRC inspection conducted on November 28-30, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in our correspondence to you dated December 31, 1974.

A.

Technical Specification 3.5 requires in part that "The reactor will not be operated unless the following equipment is operable..

Emergency Exit Manhole Hatch Open (Scram) Switch."

Contrary to the above, the Reactor Safety Committee approved a procedure revision which resulted in the reactor being operated from October 23 to November 28, 1978, with the Emergency Exit Manhole Hatch Open (Scram)

Switch inoperable.

This is an infraction.

B.

10 CFR 50.59(b) requires that the licensee maintain records of changes in the facility to the extent that such changes constitute changes in the facility as described in the safety analysis report and that these records include a written safety evaluation which provides the bases for the determination that the change, test, or experiment does not involve an unreviewed safety question.

Contrary to the above, on August 7, 1978 the pool water purification system, which is described in Section 4.7 and shown in Figure IV-I of the UVAR Revised Safety Analysis Report, was changed without a written safety evaluation to determine that the change does not involve,an I ~

unreviewed safety question.

This is an infraction.
