Docket Number 99900070/78-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on September 18-21, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplish-ed in accordance with these instructions, procedures, or drawings."
Section 12.0 of the QA Manual, paragraphs 12.1.B.4 and 12.1.C, states in part, "Any department not functioning as required shall be re-audited within thirty days to ascertain that required corrective actions have been taken A file of all audit reports, corrective action, and re-audits shall be maintained in the Quality Control Office."
Contrary to the above, there was no documented evidence in the file main-tained in the Quality Control office, that re-audits had been performed of those departments which have been established by audit to be not function-ing as required.
The inspector was informed that re-audits had been performed but had not been documented as required.
