Waterford SES Unit 3 Docket No. 50-382 Construction Permit No. CPPR-103 IE Inspection Report No. 50-382/79-03

Dear Mr. Seidle:

LP&L's response to the item of noncompliance identified in the subject report is given below:

Failure to Follow Approved Drawings - Mounting / Installation of Class IE 4160 Volt Switchgear Criterion V of Appen' dix B to 10 CFR 50 requires that activities affecting quality shall be accomplished in accordance with documen ted instructions, procedures, and drawings.

Ebasco Drawing 344-S01, Revision 5; bbnufacturer Drawing (Ebasco No.) 154 4015 R3; Design Change Notice E-172; and Field Change Requests E-390 and E-449 require that the mounting of 4160 volt switchgear 3AB3-S be accomplished as follows:

1. Using a plug weld at each of the manufacturer furn'ished mounting holes. .
2. Using a 1/4 inch, 1 inch long fillet weld between the mounting angle and the embedded mounting plate at each plug weld.

The net result of this mounting should be approximately ten plug welds and ten fillet welds around the periphery of each switchgear section attaching the manufacturer furnished mounting angle to the concrete embedded mounting channels.

The IE inspector, on March 8, 1979, observed that the mounting /

installation for the sections of 4160 volt switchgear 3AB3-S had been accomplished as follows:

1. There was no evidence of a plug weld at each of the manu- ,

facturer furnished mounting holes.

2. Four, 3 inch fillet welds (two at the front and two at the rear) were utilized. The two fillet welds at the front of each section were on the lighter gauge steel bottom pan of the switchgear instead of the mounting angle.

This mounting / installation was signed off on October 12, 1978, as acceptable by QC on Quality Control Checklist for equipment installation for 4160 volt switchgear 3AB3-S.

1. Corrective Steps Taken and The Results Achieved Nonconformance Report W3-1294 (F&M W3-133) was issued to document the nonconforming conditions on the 4160 volt switchgear 3A3-S, 3B3-S and 3AB3-S. This NCR is presently being evaluated by the Ebasco Site Support Engineering Department.
2. Corrective Steps Taken to Avoid Further Noncompliance Actions taken to preclude repetition are as follows:
a. Documented instructions were administered on April 17, 1979 and April 24, 1979 to acquaint Construction and Quality Control per-sonnel with the procedure to be followed to assure that all per-tinent documents (Engineering Drawings, Vendor Drawings, FCR's and DCN's) are thoroughly reviewed and discussed prior to per-forming installation of electrical equipment.
b. Prior to inspection of any equipment installation,the QC Inspec-tor will have a conference with the QC Engineer to' assure all pertinent documents are reviewed and discussed prior to pelform-ing the inspection on electrical equipment installation.
c. The Fischbach and Moore Quality Control Department has been re-organized to reduce the administrative load on field inspection supervision so that those individuals can spend more time in the field supervising, instructing and evaluating the performance of Field Inspectors. These changes are as follows:
1. Administrative Assistant - Added to review and approve all routine documentation in order to free the QC Supervisor from review of routine documentation so that he can spend more time in the field.

2. Other Quality Control personnel have changed positions in order to provide a more effective inspection program and supervision of inspection personnel in the field.
d. Ebasco Quality Assurance will conduct audits and surveillances in this area during the month of Fby to verify compliance by Fischbach and Moore.
3. Date When Full Compliance Will Be Achieved -

Full compliance will be achieved on May 30, 1979 by which time the nonconformance report disposition and procedure revision will have been completed.
