Appendix A NOTICE OF VIOLATION Shelwell Services, Incorporated License No. 34-10455-01 Based on the inspection conducted on December 12, 1979, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below.
Items 1 and 2 are infractions.
1.
Condition 14 of your license requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in your application dated September 26, 1978 which states, in part, that radiation survey instruments will be calibrated at six month intervals.
Contrary to the above instrument model CDV-700, serial No.
34571 had not been calibrated frcm October 27, 1978 to the day of inspection. The instrument was in recent use on truck No. 807.
2.
Condition 6 of your license limits the amount of iridium-192 that you may possess at any one time to a total of 15 millicuries.
Contrary to the above on Aubust 24, 1979 you possessed 25 milli-curies of iridium-192.
