Dear Mr. Seidle:
LP&L's response to the items of noncompliance identified in the subject report is given below:
A.
Failure to Perform Required Surveillances as Required by Approved Procedures Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be accomplished in accordance with pre-scribed instructions, procedures, and drawings.
Paragraph 6.10.1 of Ebasco Procedure ASP-IV-3, " General Housekeeping,"
requires Ebasco Quality Control personnel to perform weekly surveillances of the work areas to assure the adequacy of cleanliness and the adequacy of housekeeping practices in the work areas. These surveillances are to be documented per paragraph 6.10.1.5 of this same procedure.
Contrary to the above:
Discussions on March 22, 1979, with LP&L and Ebasco personnel confirmed that these periodic surveillances, with respect to the installed CVCS Charging Pumps, were not being performed nor documented as required by this procedure.
This is an infraction.

Failure to Follow Contract Procedures Related to Care and Maintenance of Safety Related Components Criterion V of Appendix B 10 CFR 50 requires that activities af fecting quality shall be accomplished in accordance with prescribed instructions, procedures, and drawings.
Ebasco Procedure CMI#1, " Care and Maintenance Instructions for the CVCS Charging Pumps," per paragraph D.1 requires that the charging pumps be provided with adequate protection from damage or deterioration as a result of conditions or activities in the vicinity, af ter being moved to in-place storage.
Contrary to the above:
On March 21, 1979, it was noted that, although (1) scaffolding was erected in the area of the three installed charging pumps, (2) some scaffolding structure was placed cn pumps and pump foundations, and (3) the area was cluttered with trash and debris, no protective measures existed to protect the pumps from damage or deterioration.
This is an infraction.
The above two items are considered to have a common solution described as follows:
1.
Corrective Steps Taken and The Results Achieved:
LP&L Quality Assurance Stop Work Order was issued April 9,1979 at 9:20 a.m.
to ensure the immediate removal of scaffolding materials and temporary HVAC equipment. On April 26, 1979 at 8:00 a.m. a Stop Work Rclease was issued on the basis of corrective action measures taken to protect the HPSI pumps and Charging Pumps and removal of all debris, scaf folding and temporary HVAC equipment.

Corrective Steps Taken to Avoid Further Noncompliance Actions taken to preclude repetition are as follows:'
A meeting was held on March 23, 1979 with Ebasco and LP&L personnel to address the general problems of housekeeping.
The results of this meeting initiated the following actions:
- 1) Construction will define eating areas and post signs to ensure all craf t are aware of authorized lunch eating locations.
- 2) Construction and mecurity personnel will enforce adherence to good housekeeping in authorized eating locations and all other indicated areas by roaming inspections.
- 3) Craft laborers have been instructed to remove all trash from under protective covers as well as general areas.

- 4) Protective covers were re-installed or a modified design cover was used.
- 5) Once a week a general walk-thru with contractor representatives and Ebasco supervisors will be performed to further assure that good housekeeping is maintained.
- 6) It was generally agreed that there is not a need for QC inspections and the associated paperwork to enforce and improve general house-keeping. Rather, this should be accomplished by the above actions.
Accordingly, ASP-IV-3, " General Housekeeping", will be revised to delete this requirement.

Date When Full Compliance Will Be Achieved Full compliance will be achieved on May 22, 1979 by which time all im-plementing actions will have been completed.
