Request for Additional Information Quality Assurance Program for Transportation Packaging License Numbers 45-08890-01 and 45-08890-02 Centlemen:
In response to your request for additional infortntion,Section VII " Audits" of 10CFR71 Quality Assurance Program for Transportation Packaging has been amended to allow for an audit by the Assistant Radiation Safety Coordinator or his appointees.
The Assistant Radiation Safety Coordinator has no direct responsibility in the acti-vities of the Primary Radiation Safety Of ficer.

Radiographic personnel shall perform the requires inspections and tests in accordance with established procedures. Ihe Primary Radiation Safety Officer shall insure that these functions are performed and recorded.
VI.
RECORDS Records of package approval (including references and druwings), inspections, tests, utilization logs, audit reports,, personnel training and qualifications, and shipping / receiving records will be maintained. Descriptions of equipment and written procedures will also be maintained. Records will be naintained by the Radiation Safety Officers and by Radiation Records (Primary Radiation Safety Officer).
VII. AUDITS Audits of the QA Program will be conducted at lease semi-annually by the following individuals:
A.
Primary Radiation Safety Officer B.
Assistant Radiatinn Safety Coordinator (or appointed individuals). This audit will insure that the Primary Radiation Safety Of ficer is acting in accordance with the QA Program and that all necessary records are being maintained.
The audits will be performed using a checklist which will include the following items:
A.
Are proper forma mainta ined?
B.
Are Manufacturer's Certificates available?
C.
Have proper surveys been conducted?
D.
Are shipping / receiving papers on file?
E.
Are QA and other pertinent procedures on file?
F.
Is the QA Program effective?
C.
Other Results of the audits will be maintained by the Primary Radiation Safety Officer with copies provided to the units being audited.
Should any area show a deficiency after evaluation, then the deficiency will be duly corrected and reported to the Auditor.
These audits may be conducted in conjunction with other inspections out-lined in the ADMINISTRATIVE CONTROL AND REDIOLOGICAL PROTECTION PROCEDURES.
This Quality Control Program will become a part of the overall Frochling
& Robertson, Inc. Quality Assurance Progran and will be subj ect to management review.
