(PSI) telephoned your of fice to notify you of the following two potentially reportable incidents:

1) qualification of welders ucilizing Cherne Contracting Corporation (Cherne) 'Jelding Procedure 14.6.1.77 for radiographic examination of welds.
2) Qualification of Cherne 'rielding Procedures 101BR11-4.5 and 1013021-5.0.

Regarding the first item, Cherne was performing radiographic examination of welder qualification coupons by use of a double wall exposure, single wall viewing technique with penetrameters placed on the film side of the veld, instead of on the source side. Fourteen (14) velders had been qualified on Category I piping using this procedure. ?SI questiened whether Cherne's procedure was in violation of the ASME Code requirements.

PSI decided that this item could potentially lead to extensive evaluation or repair and that the incident was reportable under 10CFR50.55(e).

Cherne is presently in the process of performing alternative welder quali-fication tests. As of June 7, 1979, eight of the original welder's coupons were requalified by bend tests in accordance with Section IX of the AS:2 Code. Another four welders will be requalified to a new Procedure 1013021-2.25 upon qualification of the procedure since their original coupons are not available. '."he remaining two welders are no longer employed by Cherne.

"" & Enforcenent one welder's original coupon is available and is in the process of being requalified by bend testing. All of the welds performed by the other welder have been cut out and rewelded.

Future welder qualifications will be performed utilizing a source side radiographic technique or bend tests.

The above constitutes an interim report as defined by 10CFR50.55(e)(3) .

PSI will provide a final report when the results of the qualification tests are available.

Regarding the second incident, two Cherne welding procedures for carbon steel pipe, 1013R11-4.5 and 1013021-5.0, were subject to question due to discrepancies in their Procedure Qualification Reports (PQR), PQR-11 and PQR-13, respectively. PSI's concern was that if the procedures were incorrectly qualified, there was a potential for extensive evaluation or corrective action and therefore was reportable under 10CFR50.55(e).

It was later determined that an additional PQR-10, which was attached to both procedures in question, was acceptable and qualified both procedures for a wall thickness up to and including 3/4". There was no welding per-formed on safety related piping for wall thicknesses above 3/4".

The above constitutes a final report on the second item and no further action will be taken by PSI.
